研究北京市老年慢病多重用药患者用药现状及不适宜性  

Objectives of Study：

To investigate the status and inappropriateness of geriatric polypharmacy of chronic diseases

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

①年龄≥65岁；②诊断包括高血压、高脂血症、冠心病、脑卒中、糖尿病中一种以上 ③使用≥5种长期（≥28天）使用药物(全身用药物)；④签署知情同意书

Inclusion criteria

1. Aged≥65 years;
2. Diagnosis includes at least one disease of hypertension, type 2 diabetes, coronary heart disease and Cerebral stroke
3. Use more than 5 Systemic medicines for more than 28 days;
4. willing to sign the Informed consent.

排除标准：

①不愿签署知情同意书的患者 ②无法配合，不能完成问卷调研的患者

Exclusion criteria：

1. unwilling to sign the Informed consent;
2. unable to finish the questionnaire survey.

The way of sharing trial data is through publishing original articles on related journals.

数据采集和管理（说明：数据采集和管理由两部分组成，一为病例记录表(Case Record Form, CRF)，二为电子采集和管理系统(Electronic Data Capture, EDC)，如ResMan即为一种基于互联网的EDC：

数据采集采用纸质CRF表进行，该文件作为源数据。数据管理采用REDcap EDC 系统。由第三方质控团队进行数据采集和管理的质控。

Data collection and Management (A standard data collection and management system include a CRF and an electronic data capture：

Paper CRF will be used as a way of raw data collecting. RED cap EDC will be used as tool for data management. An independent quality control group is in charge of whole procedure of data collection and data management.
