研究目的：

1.分析经口内镜肌切开术（POEM）手术有效性及安全性。 2.分析食管黏膜下严重纤维化的相关危险因素。 3.验证黏膜下粘连度分级预测模型的准确性、可靠性，从而实现在POEM术前预测出待切开部位食管黏膜下纤维化程度及手术实施的难度，帮助选择最合适的手术切开部位和手术方式，提高手术的成功率，减少患者创伤。  

Objectives of Study：

1. To analyze the effectiveness and safety of Peroral endoscopic myotomy (POEM) procedure. 2. To analyze the relevant risk factors for severe fibrosis of the esophageal submucosa. 3. To verify the accuracy and reliability of the prediction model for grading submucosal adhesion,.Thus, we can predict the degree of submucosal fibrosis of the esophagus and the difficulty of the operation before POEM, which help to select the most appropriate surgical incision site and surgical procedure, improve the success rate of surgery and reduce patient trauma.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1.明确诊断是贲门失弛缓；2.签署POEM手术的知情同意书；3.同意术前进行食管测压、电子胃镜检查（有完整的内镜图片）、Eckardt分级评估以及记录其它相关信息；4.已停止使用抗凝剂和抗血小板药5-7天；5.禁食48小时、限水6小时；6.年龄18-90岁

Inclusion criteria

1. Definite diagnosis of the achalasia; 2. Signing of informed consent for the POEM operation; 3. Agreement for preoperative esophageal manometry, electronic gastroscopy (with full endoscopic picture), evaluation of Eckardt grade, and recording of other relevant information; 4. Stopping anticoagulants and antiplatelet agents for 5-7 days; 5. Fasting for 48 hours, and water restriction for 6 hours;6. 18-90 years.

排除标准：

1.严重的全身性疾病，如凝血功能障碍、严重心、肺功能障碍； 2.麻醉禁忌； 3.内镜检查禁忌；4.食管黏膜表面有巨大溃疡或其它严重影响评估结果的情况；5.孕妇或哺乳期妇女。

Exclusion criteria：

1. Severe systemic diseases, such as coagulopathy, severe heart and lung dysfunction; 2. Anesthesia contraindication; 3. Endoscopy contraindicatiom; 4. Huge ulcers on the mucosal surface of the esophagus or other conditions which can seriously affect results of the evaluation; 5. Pregnant or lactating women.
