研究目的：

探究中国热带地区热应激肾病的临床特征、肾脏病理特征危险因素和预后分析，针对在高温高湿环境下从事体力劳动者进行肾脏功能异常的筛查，采用病例对照研究的方式明确热应激肾病相关的危险因素，进而制定具有中国特色的热应激肾病防治指南。  

Objectives of Study：

To explore the clinical features, kidney pathological characteristics, risk factors and prognosis of heat stress-induced nephrology (HSN) in Chinese tropical zone, kidney dysfunction screening of manual laborers in high temperature and high humidity environment was conducted. HSN-associated risk factors were investigated using case-control study, aiming to formulate the prevention and control guidelines of HSN in Chinese tropical zone.

Description for medicine or protocol of treatment in detail：

 

纳入标准：

疑似热应激肾病组：1、年龄18-60岁；2、生活于热带地区，时间>1年，在高温高湿的环境下从事体力工作；3、有肾脏损害的表现>3个月以上，包括但不局限于eGFR<60 ml/min/1.73m2，或24h尿蛋白水平在300mg至3g，或尿常规尿蛋白定性在30-300mg/dl，或肾脏病理/尿常规结果提示肾间质小管损伤（肾小管酸中毒，肾性失钾，肾性糖尿等）。
正常对照组：与热应激组生活环境、性别及年龄特征相似，未伴有肾脏功能异常的人员。

Inclusion criteria

Suspected heat stress nephropathy group: 1. Age between 18-60; 2. Living in tropical regions, time longer than 1 year, engaged in physical work in a high temperature and high humidity environment; 3. With the manifestation of renal damage more than 3 months, including but not limited to: eGFR larger than 60 ml/min/1.73m2, or 24h urine protein level between 300mg to 3g, or routine urinary protein characterization of 30-300mg/dL, or renal pathology/routine urine results suggest renal Interstitial tubular injury (renal tubular acidosis, renal potassium loss, renal glycourine, etc.).
Normal control group: subjects with similar living environment, gender and age characteristics as the heat stress group, without abnormal renal function.

排除标准：

1、有肾脏基础疾病，如多囊肾，先天畸形，肾积水，诊断明确的肾小球疾病（如FSGS、膜性肾病等），系统性疾病：自身免疫病，血液系统疾病，血管炎，血液及肿瘤系统疾病等，诊断明确代谢性疾病导致肾脏损害的，如糖尿病、高血压等导致肾脏损害者（需要肾脏病理结果支持）；2、24h尿蛋白水平>3g，或尿常规尿蛋白定性>300mg/dl；3、中暑或热射病导致肾功能一过性损伤但短期恢复正常者。

Exclusion criteria：

1. With clear diagnosis of basic renal diseases, such as polycystic kidney, congenital malformation, hydronephrosis, clearly diagnosed glomerular diseases (such as FSGS, membranous nephropathy, etc.), systemic diseases: autoimmune diseases, blood system diseases, vasculitis, blood and tumor system diseases, etc., with clear diagnosis of metabolic diseases leading to kidney damage, such as diabetes, hypertension and other kidney damage (with renal pathological results); 2. 24h urinary protein level larger than 3g, or urine routine urine protein determination larger than 300 mg/dl; 3. The transient renal function injury caused by heat stroke or heat stroke disease but returned to normal in a short time.
