研究目的：

本项目主要针对老年腰椎间盘突出症围手术期的病人，运用柯氏培训评估模式（Kirkpatrick Model）的健康教育模式对老年腰椎围手术期的病人进行健康教育及健康教育效果的评估，通过柯氏评估来了解患者对健康教育的满意度及掌握情况，并根据患者的意见和建议对健康教育的内容和形式进行改进、完善后进行再次宣教，使患者对健康教育的内容完全掌握，提高患者围手术期对护理的依从性，减少术后并发症；另一方面通过反应层的评估，根据患者的意见及建议对现有的健康教育模式进行改进，制定出适合老年人的个性化的健康教育模式，从而提高老年患者围手术期对护士的依从性，减少术后并发症的发生率，减少住院天数，降低医疗费用。  

Objectives of Study：

This project is mainly aimed at elderly perioperative patients with lumbar disc herniation. The health education model of the Kirkpatrick Model is used to evaluate the health education and health education effects of senile lumbar perioperative patients. Assessment to understand the patient's satisfaction with the health education and grasp the situation, and according to the patient's opinions and recommendations on the content and form of health education to improve and perfect after the re-education, so that patients fully grasp the content of health education, improve patient care Compliance with care during the operative period reduces postoperative complications; on the other hand, through the evaluation of the response layer, the existing health education model is improved according to the patient's opinions and recommendations, and a personalized health education suitable for the elderly is developed. The model can improve the compliance of elderly patients with nurses during the perioperative period, reduce the incidence of postoperative complications, reduce hospitalization days, and reduce medical expenses.

药物成份或治疗方案详述：

试验组采用柯氏培训评估模式的健康教育模式，采用图文并茂的形式对患者进行围手术期的健康教育，其内容包括术前晚饮食的要求、术后进食水的时间，术后翻身的时间、术后床上排便的方法及术后腹部按摩频率；其次，对健康教育效果进行评估，健康教育效果评估共分为四个阶段：（1）反应评估（Reaction），（2）学习评估（Learning），（3）行为评估（Behavior）（4）成果评估（Result），评估后针对患者提出的意见进行改进并对健康教育内容掌握不全面的地方再次进行宣教；对照组仅采用常规的健康教育的方法对围手术期的患者进行健康教育。对两组患者术后肠功能恢复的时间进行研究。 

Description for medicine or protocol of treatment in detail：

The experimental group used the Kosher training evaluation model of health education, using pictures and texts to provide perioperative health education to the patients. The contents included preoperative dietary requirements, postoperative water intake time, and postoperative postures. The method of postoperative defecation on the bed and the frequency of postoperative abdominal massage; secondly, the evaluation of health education effects, and the evaluation of health education effects are divided into four phases: (1) Reaction assessment, (2) Learning assessment (Learning) , (3) Behavior assessment (4) Result assessment, improvement of opinions proposed by patients after assessment, and re-education of areas where health education content is incomplete; the control group adopted only regular health education Methods Health education for perioperative patients. The time of recovery of bowel function after surgery in both groups was studied. 

纳入标准：

① 年龄≥60岁的患者；
② 同意并签署知情同意书的患者；
③ 腰椎择期手术患者

Inclusion criteria

1. patients aged 60 years or older;
2. Patients who agree and sign the informed consent form;
3. patients undergoing elective lumbar surgery

排除标准：

① 心肾功能不全；
②伴有胃肠道相关器质性病变的患者
③ 术前低钾血症、腹水、腹膜炎、肠梗阻病史的患者
④伴有精神、意识障碍相关病症的患

Exclusion criteria：

1. heart and kidney insufficiency;
2. Patients with gastrointestinal related organic lesions;
3. Patients with a history of preoperative hypokalemia, ascites, peritonitis, and intestinal obstruction;
4. Suffering from disorders related to mental and consciousness disorders.

A total of 120 surgical patients who met the inclusion criteria for lumbar disc herniation in the spine orthopedic department of the Gansu Provincial Hospital of Traditional Chinese Medicine were included in the study.
