研究目的:

慢性非特异性下腰痛(Chronic Nonspecific Low Back Pain,CNLBP)是临床多发病及常见病,属于中医筋 伤病中“腰肌劳损”范畴,本病的发生与腰部软组织损伤密切相关。椎旁肌群在本病的发生和发展中起着重 要的作用,它们形态和电生理的异常是导致CNLBP的一个重要原因。中医理筋手法是CNLBP最常用的保守治疗 方法,也是循证医学推荐的治疗方法之一。其治疗CNLBP具有良好的临床疗效,前期相关临床研究发现,理 筋手法能够较好地调整腰部肌肉张力、缓解肌肉疲劳、增进血液循环、加速代谢速率,可有效恢复椎旁肌肉 的结构和功能,进而从根本上减缓本病的发生及发展。 目前对于理筋手法治疗CNLBP缺少高等级的随机对照临床研究,且缺少对于椎旁肌群结构及功能影响的相关 研究。在此背景下,本课题组拟通过理筋手法治疗CNLBP的前瞻性随机对照临床试验,运用MRI影像学及表面 肌电指标对CNLBP患者椎旁肌群在手法干预前后分别进行检测评价,观察和评价各指标的变化情况,用以评 价治疗效果,以期验证理筋手法治疗CNLBP的临床疗效,并探讨理筋手法对于CNLBP患者椎旁肌群的形态学及 电生理特性的影响。  

Objectives of Study:

Chronic nonspecific low back pain (cnlbp) is a frequently occurring and common disease in clinic, which belongs to TCM The occurrence of this disease is closely related to the injury of lumbar soft tissue. Paravertebral muscles play an important role in the occurrence and development of this disease The abnormal morphology and electrophysiology of cnlbp play an important role in the pathogenesis of cnlbp. Traditional Chinese medicine tendon manipulation is the most commonly used conservative treatment of cnlbp It is also one of the treatment methods recommended by evidence-based medicine. Its treatment of cnlbp has a good clinical effect, early clinical studies found that the reason Tendon manipulation can better adjust lumbar muscle tension, relieve muscle fatigue, improve blood circulation, accelerate metabolic rate, and effectively restore paravertebral muscles In order to slow down the occurrence and development of the disease. At present, there is a lack of high-level randomized controlled clinical research on the treatment of cnlbp with tendon manipulation, and there is a lack of relevant research on the structure and function of paravertebral muscles Research. Under this background, our group plans to conduct a prospective randomized controlled clinical trial of tendon manipulation in the treatment of cnlbp, using MRI imaging and surface treatment Electromyography indexes were used to detect and evaluate the paravertebral muscles of cnlbp patients before and after manipulation intervention, and the changes of each index were observed and evaluated to evaluate the effect of manipulation intervention In order to verify the clinical efficacy of tendon manipulation in the treatment of cnlbp, and to explore the effect of tendon manipulation on the morphology and morphology of paravertebral muscles in patients with cnlbp The influence of the electrophysiological characteristics.

纳入标准:

①年龄 30-60 岁;
②确定诊断为非特异性下腰痛患者;
③既往未接受过腰部手术治疗;
④对中医正骨手法无抵触感;
⑤自愿参加试验,并签署相关知情同意书。

Inclusion criteria

① They were 30-60 years old;

② The patients with nonspecific low back pain were confirmed;

③ No previous lumbar surgery was performed;

④ It has no touch to the bone setting manipulation of traditional Chinese medicine;

⑤ Volunteer to participate in the trial and sign the relevant informed consent.

排除标准:

①患有椎间管狭窄、腰椎间盘突出;
②拒绝选择以上治疗方案或不理解该试验者;
③治疗疗程中要求临时退出者;
④精神障碍者。

Exclusion criteria:

① Patients with lumbar spinal stenosis, lumbar disc herniation;

② Those who refused to choose the above treatment or did not understand the trial;

③ Those who asked to withdraw temporarily during the course of treatment;

④ Mental disorders.
