研究目的：

目前重症患者的营养支持仍是当代临床研究的热点兼难点之一。无论是营养供给不足抑或过度喂养均会影响患者的预后。营养供给不足时，机体过度消耗，免疫功能下降，甚至引起肌无力，特别是引起呼吸肌收缩无力，从而直接导致机械通气脱机困难。过度喂养又会导致心、肺和肝脏等器官的代谢并发症，影响患者的预后。由此可见，早期给予适度的营养治疗可以改善重症患者的预后。应用传统的预测公式及基于体重的简化公式（25-30Kcal/kg/day）指导重症患者的营养支持治疗，无法精准评估从而实现个体化治疗。而目前基于静息能量消耗数据可以更精准的评估患者的营养需求。这项技术主要是应用间接测热法（IC）原理自动计算出能量代谢数据，而IC被认为是目前评估能量消耗（EE）的金标准，通过对重症患者营养需求精准的评估为临床医师对患者营养支持治疗的管理提供更好的指导，改善患者的预后。 本项目立足于进一步优化重症患者营养支持治疗的临床救治原则，将基于间接测热法（IC）测定能量代谢数据指导重症患者营养支持治疗技术应用于重症患者营养支持的临床实践中，并观察其治疗效果，以期提高重症患者的救治成功率。 本项目立足于进一步优化重症ARDS患者营养支持治疗的临床救治原则，将基于间接测热法（IC）测定能量代谢数据指导重症患者营养支持治疗技术应用于重症患者营养支持的临床实践中，并观察其治疗效果，以期提高重症患者的救治成功率。  

Objectives of Study：

This project based on the further optimization of severe ARDS patients clinical treatment principles of nutritional support treatment, will be based on indirect calorimetry (IC) determination of energy metabolism measurement data to guide the intensive nutritional support in patients with nutritional support treatment technology applied in the severe patients in clinical practice, and to observe the treatment effect, so as to enhance the successful rate of critically ill patients.

肠内营养及肠外营养 

Description for medicine or protocol of treatment in detail：

Enteral nutrition and parenteral nutrition 

纳入标准：

选择研究期内入住ICU行机械通气的重症患者30例，纳入标准包括：（1）符合机械通气适应证，如缺乏保护性气道反射；（2）临床资料完整；（3）年龄18～65岁；（4）入住ICU时间≥24ｈ；（5）气管插管机械通气；（6）NRS 2002评分≥5分或NUTRIC评分（不纳入IL-6）≥5分。

Inclusion criteria

Thirty critically ill patients admitted to ICU for mechanical ventilation during the study period were selected. Inclusion criteria included :(1) meeting indications of mechanical ventilation, such as lack of protective airway reflex;(2) Complete clinical data;(3) Age 18 to 65;(4) Stay in ICU ≥24h;(5) Endotracheal intubation mechanical ventilation;(6) NRS 2002 score ≥5 or NUTRIC score (not including IL-6) ≥5.

排除标准：

排除标准包括：（1）呼吸机管道漏气＞10％；（2）肛温＞38.5℃，高热；（3）肾衰进行透析、血液净化治疗；（4）合并糖尿病、甲亢等代谢性疾病；（5）脑死亡；（6）机械通气氧浓度＞60％；（7）需要频繁调整血管活性药物；⑧躁动不安，无法配合检查。入选患者在ICU内接受基于静息能量消耗监测数据指导的营养支持治疗，同时采取分别针对病因的解除治疗和以器官功能维护为中心的综合治疗，评价基于间接测热法测定的能量代谢数据指导重症患者的营养支持治疗的治疗效果。

Exclusion criteria：

Exclusion criteria include :(1) ventilator pipeline leak > 10%;(2) Anal temperature BBB 0 38.5℃, high fever;(3) dialysis and blood purification for renal failure;(4) Metabolic diseases such as diabetes and hyperthyroidism;(5) brain death;(6) Mechanical ventilation oxygen concentration > 60%;(7) Frequent adjustment of vasoactive drugs is needed;⑧ restless, unable to cooperate with the examination.The enrolled patients received nutritional support treatment guided by resting energy consumption monitoring data in the ICU, and at the same time, dissolving treatment for etiology and comprehensive treatment centered on organ function maintenance were adopted to evaluate the therapeutic effect of nutritional support treatment guided by energy metabolism data measured by indirect calorimetry in critical patients.
