研究目的：

观察“补脾益肾方”对CKD4期患者肾小球滤过率下降速率和终点事件发生率的影响，评价其对CKD4期的疗效和安全性  

Objectives of Study：

To test the hypothesis that Chinese medicine "Bu Pi Yi Shen Formula" is superior to Losartan in slowing down the decrease of glomerular filtration rate (GFR) and reducing the end-point events in patients with stage 4 CKD. To estimate the safety of “Bu Pi Yi Shen Formula” on patients with CKD 4.

药物成份或治疗方案详述：

中医组治疗方案：补脾益肾方颗粒+氯沙坦安慰剂+基础治疗；氯沙坦组治疗方案：补脾益肾方安慰剂颗粒+氯沙坦+基础治疗； 

Description for medicine or protocol of treatment in detail：

Components of Bupi Yishen Formula (BYF) include Astragalus mongholicus (Huangqi), Codonopsis pilosula (Dangshen), Macrocephalae (Baizhu), Poria cocos (Fuling), Rhizoma dioscoreae (Shanyao), Semen coici (Yiyiren), Radix polygoni multiflori (Heshouwu), Semen cuscutae (Tusizi), and Salvia miltiorrhiza (Danshen). In the BYF group, patients will be instructed to dissolve a BYF sachet in 150 ml of boiled water and to take this solution orally three times daily and a losartan matched placebo daily for 1 year. Patients in the losartan group will be instructed to take 100 mg losartan orally once daily and also a placebo sachet dissolved in 150 ml of boiled water three times daily for 1 year. 

纳入标准：

①18～80岁；
②符合慢性肾脏病4期诊断标准并且3个月内GFR 15～29.9ml/min/1.73m2；
③符合脾肾气虚证的诊断标准
④血压＜140/90mmHg

Inclusion criteria

A subject will be eligible for inclusion in the trial only if all the following criteria are fulfilled at baseline:
1. Aged 18~80 years;
2. CKD stage 4 (GFR 15-29.9ml/min/1.73m2), during the 3 months before inclusion;
3. With the Chinese Medicine syndrome 'Spleen and Kidney Deficiency';
4. Blood pressure <140/90mmHg.

排除标准：

①立即需要透析治疗?、正在接受激素、非甾体类消炎药或免疫抑制剂治疗；
②严重心律失常、急性心衰、心肌梗死、急性脑卒中、不稳定性心绞痛、NYHA分级Ⅳ级者（纽约心脏病协会心功能分级标准）；
③合并急性感染性疾病、活动期恶性肿瘤、慢性乙型肝炎活动期及迁延期、肝硬化失代偿期或造血系统等严重原发性疾病、糖尿病、活动性结核或精神病患者；
④入组前3个月内 GFR波动范围≥30％；
⑤血钾高于参考区间上限；
⑥已知对该研究中某种药物过敏或无法耐受者；
⑦孕妇或哺乳期患者；
⑧正在参加其他临床试验的患者；
⑨非黄种人。

Exclusion criteria：

A subject will be excluded if anyone of the following criteria is fulfilled at baseline:
1. Urgent need for dialysis; Undergoing glucocorticoid, NSAIDS or other immunosuppressive therapy;
2. Serious arrythmia, heart failure (IV in NYHA classification), myocardial infarction, unstable angina or cerebrovascular accident;
3. Combined with disease as diabetes, malignant tumor at active stage; hepatitis B in active and delay moved; liver cirrhosis in decompensation; disease of hematopoietic system; active tuberculosis; acute infectious disease;
4. fluctuation of GRF >=30% in the 3 months before inclusion;
5. Hyperkalemia(Potassium is higher than reference interval upper limit);
6. Allergic history of any ingredient of the drugs or herbs used in this therapy;
7. Pregnant or lactating;
8. Admission to other clinical trials at the time;
9. Non yellow race.

盲法：

为保证临床研究实施过程中双盲的实现，临床研究者、受试者、监查员、数据管理员以及统计分析人员均不知道试验处理及药物分配情况，即不知道哪一个受试者分配至哪一组使用哪一种药物，本试验药物编盲与随机化同时进行操作。 药物编号盲底由广东省中医院中医药临床研究方法学重点研究室人员保管，临床医生不应知道盲底。当一名病人被纳入时，研究者登陆网上随机分配系统，取得药物编号。

Blinding：

As this is a double-blind study, the following persons will be blinded: -patients; -researchers and their study related staff; -statistician; - monitoring members. The randomization list will be kept strictly confidential. Only the staff in DME Center who generated the randomization list have access to the randomization list.
