研究心脏MRI在心肌淀粉样变患者诊断、预后预测和监测治疗反应价值。  

Objectives of Study：

To study the value of cardiac MRI in the diagnosis, prognosis prediction, and monitoring treatment response of patients with myocardial amyloidosis.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1.需具备诊断AL型淀粉样变的条件：血/尿免疫固定电泳可见单克隆条带，血/尿游离轻链升高；骨髓涂片可见异常浆细胞增生，流式细胞学判定为限制性表达轻链的单克隆浆细胞；累及器官活检可见刚果红染色阳性、免疫荧光可见轻链阳性;
2.诊断的AL型淀粉样变患者，根据NCCN指南上心脏累及诊断标准：心脏超声平均心室壁厚度>12mm，排除其他心脏疾病；或在没有肾功能不全及心房颤动时NT-proBNP>332ng/L;
3.经心肌活检诊断确诊淀粉样变；
4.18岁≤年龄≤75岁，男女不限。

Inclusion criteria

1. The conditions for diagnosing AL amyloidosis are required: monoclonal bands can be seen in blood/urine immunofixation electrophoresis, blood/urine free light chains are elevated; abnormal plasma cell proliferation can be seen in bone marrow smears, and flow cytometry is judged to be restricted Monoclonal plasma cells that express light chain sexually; biopsy of the involved organs shows positive staining of Congo red and positive light chain in immunofluorescence.
2. For diagnosed AL amyloidosis patients, according to the diagnostic criteria of cardiac involvement in the NCCN guidelines: the average ventricular wall thickness on echocardiography> 12mm, excluding other heart diseases; or NT-proBNP > 332ng/L in the absence of renal insufficiency and atrial fibrillation.
3. Diagnosis of amyloidosis by myocardial biopsy.
4. Aged 18 to 75 years, regardless of gender.

排除标准：

1.确诊后不接受化疗方案治疗；
2.CMR检查常规禁忌症（如起搏器／ICD植入／幽闭恐惧症／肾功能不全／MR对比剂过敏）。

Exclusion criteria：

1. Do not receive chemotherapy treatment after diagnosis;
2. CMR examination routine contraindications (such as pacemaker/ICD implantation/claustrophobia/renal insufficiency/MR contrast agent hypersensitivity).
