研究目的：

该研究旨在研究小儿推拿治疗小儿功能性便秘的疗效及安全性。  

Objectives of Study：

This study aims to evaluate the efficacy and safety of pediatric Tuina for children with FC.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1. 年龄为1-7岁的小儿；
2. 符合功能性便秘(Rome IV)诊断标准的儿童：
（1）4岁以下小儿1个月内至少出现两种以下情况：
1)每周排便2次或更少；
2)大便潴留过多史；
3)有大便痛或排便困难的病史；
4)大直径大便史；
5)直肠内有一大块粪便。在接受过如厕训练的儿童中，则需以下附加标准：
6）在获得如厕技能后，每周至少有1次大便失禁；
7)有可能堵塞马桶的大直径大便史。
（2）4岁以上儿童,一个月内必须出现2个或更多的症状，且不满足肠易激综合症的诊断标准：
1)每周2或更少的在厕所排便；
2)每周至少1次大便失禁；
3)有保持姿势或过度的大便潴留史；
4)有大便痛或排便困难的病史；
5)直肠内有较大粪便团块；
6)有可能堵塞马桶的大直径大便史；
3.父母或法定监护人主观同意参与试验；
4. 父母或法定监护人签署书面知情同意书。

Inclusion criteria

1. Children aged 1-7 years;
2. Children who meet the diagnostic criteria for functional constipation (Rome IV);
(1) Must include 1 month of at least 2 of the following in infants up to 4 years of age:
1) 2 or fewer defecations per week;
2) History of excessive stool retention;
3) History of painful or hard bowel movements;
4) History of large-diameter stools;
5) Presence of a large fecal mass in the rectum. In toilet-trained children, the following additional criteria may be used:
6) At least 1 episode/week of incontinence after the acquisition of toileting skills;
7) History of large-diameter stools that may obstruct the toilet.
(2) For children over 4 years of age, there must include 2 or more of the following occurring at least once per week for a minimum of 1 month with insufficient criteria for a diagnosis of irritable bowel syndrome:
1) 2 or fewer defecations in the toilet per week in a child of a developmental age of at least 4 years;
2) At least 1 episode of fecal incontinence per week;
3) History of retentive posturing or excessive volitional stool retention;
4) History of painful or hard bowel movements;
5) Presence of a large fecal mass in the rectum;
6) History of large diameter stools that can obstruct the toilet;
3. Parents or legal guardians are motivated to participate in the trial;
4. Parents or legal guardians' sign was written informed consent.

排除标准：

1. 有肠道手术史；
2. 在纳入前14天接受胃肠促动力剂或泻药治疗；
3. 入组前14天接受小儿推拿或益生菌治疗；
4. 患有先天性巨结肠病的儿童；
5. 患有神经系统和精神疾病(如脑瘫、自闭症、脊柱裂、神经性厌食症)的儿童；
6. 患有造血、内分泌、免疫系统的器质性或代谢性疾病的儿童。

Exclusion criteria：

1. A history of intestinal surgery;
2. Receiving gastrointestinal prokinetic agent or laxative treatments 14 days before enrollment;
3. Receiving pediatric Tuina or probiotics 14 days before enrollment;
4. Children with Hirschsprung's disease;
5. Children with neurological and psychiatric disorders (e.g., cerebral palsy, autism, spina bifida, anorexia nervosa);
6. Children with any organic or metabolic diseases of hematopoietic, endocrine, immune systems.

符合条件的儿童将按照1:1的比例被随机分成两组。一个独立的成员将通过云南中医药大学临床研究中心的计算机随机数发生器生成随机序列，并将随机数字指定为推拿组或对照组。

Randomization Procedure (please state who generates the random number sequence and by what method)：

Eligible FC children will be randomly assigned into two groups at 1:1 ratio. The randomization will be conducted by using a computerized random number generator, the Clinical Research Center of Yunnan University of Chinese Medicine. The random numbers were assigned to the pediatric Tuina group or control group.

该研究采用了两种不同的干预方式。无法实现对治疗师、患儿父母或监护人的盲分组。但可实现检测员及数据分析员的盲法。

Blinding：

There are two different types of interventions in this study. It is impossible to blind therapists and childrens parents or legal guardians. Nonetheless, outcome assessors and the data analyst will be blinded to the procedure and results of randomization, group allocation and intervention.
