研究目的：

评价粪菌移植治疗难治性糖尿病患者的安全性和有效性。  

Objectives of Study：

To assess the efficiency and safety of fecal microbiota transplantation in the treatment of refractory diabetic patients.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1.年龄在18-75岁。2.符合1型糖尿病诊断标准，其患者经胰岛素治疗6月以上未能有效稳定控制血糖，表现为空腹血糖日差>5.55mmol/l；日内血糖波动>11.1mmol/l；尿酮体阳性多次发生；难以预料低血糖事件是否发生。3.符合2型糖尿病诊断标准，其患者使用胰岛素用量≥0.8U/kg·d和（或）联合口服降糖药治疗6月以上，血糖仍控制不好。4.自愿参加且能配合医生开展研究，并签署知情同意书。

Inclusion criteria

1. Aged from 18 to 75 years;
2. Diagnosis of type 1 diabetes whose glycemia is poorly controlled with insulin treatment for more than 6 months. Manifested as follows: FPG fluctuation >5.55mmol/L; 2h-PG fluctuation ≥11.1mmol/L; urinary ketone body positive repeatedly; hypoglycaemic events occur unpredictably;
3. Diagnosis of type 2 diabetes whose glycemia is poorly controlled with insulin dose over 0.8U/kg.d and/or combining oral antidiabetic drugs more than 6 months;
4. Patients volunteered to participate in and sign the consent form.

排除标准：

1.发生严重糖尿病并发症的患者，如严重视网膜病变、腿部溃疡等；2.既往有胃或小肠手术导致胃肠道解剖结构改变的疾病；3.合并其他自身免疫性疾病，如炎症性肠病、系统性红斑狼疮等；4.妊娠期或哺乳期妇女；5.怀疑或确有药物滥用者；6.酗酒者：每天饮用含酒精饮品＞2份或每周＞14份的受试者（一份酒精饮品定义为150ml的葡萄酒，或350ml啤酒，或50ml标准度数80%（40度）的烈酒）；7.有严重食物过敏史；8.患有严重器质性疾病，如肿瘤、冠心病、中风等；9.过去3月内以任何药物方法或手术方式进行减肥者；10.在入组前3月内使用过抗生素并持续3天以上，入组前4周内使用过益生菌制剂；11.经研究者判断，不适宜参加临床研究者或不能配合完成研究者。

Exclusion criteria：

1. severe diabetic complications,such as severe retinopathy,ulcer,etc;
2. had gastric or intestinal surgery which caused changes in the anatomy of the gastrointestinal tract;
3. had other autoimmune diseases,such as inflammatory bowel disease,systemic lupus erythematosus,etc;
4. patients with pregnancy or having a fertility plan;
5. patients with drugs or other substance abusers;
6. Alcoholics:Subjects consuming> 2 servings of alcoholic beverages or> 14 servings per day (one Alcoholic beverages are defined as 150ml of wine, or 350ml of beer, or 50ml The standard degree of 80% (40 degrees) of spirits);
7. Have a history of severe allergy;
8. patients with severe heart,liver,renal insufficiency,severe cerebrovascular and lung disease;
9. reduce weight by any drugs or surgery;
10. participants who taking antibiotics within 3 months or probiotics within 4 weeks;
11. patients can not cooperate with clinical researchers for other reasons.
