研究目的：

主要目的：比较上肢神经入肌点选穴与阳明经选穴的临床疗效和安全性。  

Objectives of Study：

Main purpose: To compare the clinical efficacy and safety of nerve entering muscle point selection and Yangming Meridian point selection in upper extremity.

药物成份或治疗方案详述：

临床纳入就诊于绵竹市人民医院康复医学科符合诊断标准及纳入标准的脑卒中上肢功能障碍的患者100人。随机分为试验组和对照组各50人。试验组采取针刺方案：肩髃、肩髎、肩中穴、臑会、手三里、三阳络、四渎。对照组选穴方案：肩髃、曲池、手三里、合谷（参照全国中医药行业高等教育“十二五”规划教材《针灸治疗学》中风篇的选穴）。针刺得气后，电针刺激，频率为2HZ疏密波。疗程：留针30min次/日，5次/周，共治疗4周。观察两组患者治疗前后临床疗效比较，上肢肢体运动功能评定标准（FMA）、肌力、肌张力日常生活功能、表面肌电检查比较。进行统计分析，做出疗效及安全性评定。 

Description for medicine or protocol of treatment in detail：

100 patients with stroke upper limb dysfunction were admitted to the Department of Rehabilitation Medicine of Mianzhu People’s Hospital. The patients were randomly divided into experimental group and Control Group (50 persons each) . The experimental group was given acupuncture schemes: Jianyu, Jianliao, Jianzhong acupoint, Jianhui, Shousanli, Sanyangluo and Sidu. The scheme of acupoint selection in the control group was Jianyu, Quchi, Shousanli and Hég? L.I. 4(referring to the point selection in the middle part of the “12th five-year plan”textbook acupuncture and moxibustion therapeutics) . The frequency of electro-acupuncture stimulation was 2 HZ rarefaction wave. Treatment Course: Keep Needle 30 Min Times per day, 5 times per week for 4 weeks. The clinical effects of the two groups were compared before and after treatment. FMA, muscle strength, muscle tension, daily life function and surface electromyography were compared. Statistical analysis was performed to evaluate the efficacy and safety. 

纳入标准：

1.研究对象符合脑卒中诊断标准；
2.年龄在40-75岁之间；
3.病程：发病2周-6个月；
4.所有患者均经过MRI或CT检查确诊为首次发生脑梗死或脑出血；
5.无认知障碍，无感觉性失语，能够理解基本指令，MMSE评分大于等于21分；
6.自愿参加，并完成试验；
7.上肢Brunnstrom分期Ⅰ-Ⅳ期；
8.所有患者均签署知情同意书。

Inclusion criteria

1. Subjects meets stroke diagnostic criteria;
2. Aged 40 to 75 years;
3. Course of disease: onset of 2 weeks to 6 months;
4. All patients were diagnosed with cerebral infarction or cerebral hemorrhage for the first time after MRI or CT examination;
5. No cognitive impairment, no sensory aphasia, able to understand basic instructions, MMSE score greater than or equal to 21 points;
6. Volunteer to participate and complete the test;
7. Brunnstrom staging of upper limbs I-IV;
8. All patients signed an informed consent form.

排除标准：

2.蛛网膜下腔出血、小脑功能障碍或其他神经系统疾病；
3.合并其他影响肢体活动功能的疾病者，如颈腰椎疾病、骨关节病、类风湿关节炎等；
4.有肩关节炎、既往肩关节外伤者；
5.伴严重内脏疾病者；
6.上肢有严重痉挛或疼痛、或肩关节脱位、肩手综合征等；
7.出血类疾病非针刺适应症。

Exclusion criteria：

1. Subarachnoid hemorrhage, cerebellar dysfunction or other neurological diseases;
2. Patients with other diseases that affect the function of limbs, such as cervical and lumbar spine diseases, osteoarthritis, rheumatoid arthritis, etc;
3. People with shoulder arthritis and previous shoulder joint trauma;
4. Those with severe visceral disease;
5. Severe spasm or pain in the upper limbs, or dislocation of the shoulder joint, shoulder-hand syndrome, etc;
6. Non-acupuncture indications for bleeding diseases.
