研究目的：

收集不同证型干眼病人的泪液，运用蛋白组学及代谢组学等现代生物学技术，研究肺阴不足证、肝经郁热证、气阴两虚证、邪热留恋证4种不同证型干眼的差异蛋白质及代谢物，并通过数据挖掘分析导致4种不同证型干眼与健康者差异的生物学本质，探索不同证型干眼的有效生物标志物，为不同证型干眼诊断的客观化、标准化提供临床指导，为不同证型干眼靶向蛋白的药物研发提供理论基础。  

Objectives of Study：

Using proteomics, metabonomics and other modern biological techniques, we collected the tears of patients with dry eye of different syndrome types, and studied the differential proteins and metabolites of dry eye of four different syndrome types: lung yin deficiency syndrome, liver meridian stagnant heat syndrome, Qi and yin deficiency syndrome, and pathogenic heat retention syndrome, Objective to explore the effective biomarkers of dry eye of different syndrome types, provide clinical guidance for the objectivity and standardization of diagnosis of dry eye of different syndrome types, and provide theoretical basis for drug research and development of targeted proteins of dry eye of different syndrome types.

Description for medicine or protocol of treatment in detail：

 

纳入标准：

西医诊断：参考《中国干眼专家共识：检查和诊断（2020年）》：①患者主诉有眼部干涩感、异物感、烧灼感、疲劳感、不适感、眼红、视力波动等主观症状之一，中国干眼问卷量表 ≥ 7 分或眼表疾病指数( ocularsurface disease index, OSDI ) ≥13 分；同时患者 FBUT ≤ 5 s 或 NIBUT < 10 s 或 Schirmer Ⅰ试验（无麻醉）≤ 5 mm/5 min，可诊断干眼。②患者有干眼相关症状，中国干眼问卷量表≥ 7分或 OSDI ≥ 13分；同时，患者 FBUT >5 s且≤ 10 s或NIBUT 为10~12 s，SchirmerⅠ试验（无麻醉）> 5 mm/5 min 且≤10 mm/5 min，则须采用荧光素钠染色法检查角结膜，染色阳性（≥5个点）可诊断干眼。
中医诊断：参照彭清华主编2016年出版全国中医药行业“十三五”规划教材《中医眼科学》，制订干眼各证型的诊断依据：①肺阴不足证：主症：眼干涩不爽，不耐久视，白睛如常或稍有赤脉，黑睛可有细点星翳，反复难愈；次症：口干鼻燥，咽干，便秘；舌脉：苔薄少津，脉细无力。②肝经郁热证：主症：目珠干涩，灼热刺痛，或白睛微红，或黑睛星翳，或不耐久视；次症：口苦咽干，烦躁易怒，或失眠多梦，大便干或小便黄；舌脉：舌红，苔薄黄或黄厚，脉弦滑数。③气阴两虚证：主症：目内干涩不爽，目燥乏泽，双目频眨，羞明畏光白睛隐隐淡红，不耐久视，久视后则诸症加重；次症：口干，唇燥裂，神疲乏力。舌脉：舌质红，脉细弱。④邪热留恋证：主症：患风热赤眼或天行赤眼之后期，微感畏光流泪，有少许眼眵，干涩不爽，白睛少许赤丝细脉而迟迟不退，睑内亦轻度红赤；舌脉：舌质红，苔薄黄，脉数。以上各证型具备主症及次症2项或者以上，并参照舌脉，即可辨证为干眼不同证型。
纳入标准：受试患者须符合上述诊断标准，并具备以下条件：①年龄18～75 岁；②无药物过敏史；③签署知情同意书。

Inclusion criteria

Diagnosis of Western medicine: refer to "Chinese expert consensus on dry eye: examination and diagnosis (2020)": ① patients complained of one of the subjective symptoms, such as dry feeling, foreign body sensation, burning sensation, fatigue, discomfort, redness, vision fluctuation, etc., with China dry eye questionnaire score ≥ 7 or ocular surface disease index (OSDI) ≥ 13; At the same time, patients with Fbut ≤ 5 s or nibut < 10 s or Schirmer I test (no anesthesia) ≤ 5 mm / 5 min can be diagnosed as dry eye. ② The patient had dry eye related symptoms, with Chinese dry eye questionnaire score ≥ 7 or osdi score ≥ 13; At the same time, patients with Fbut > 5 s and ≤ 10 s or nibut of 10 ~ 12 s, Schirmer I test (no anesthesia) > 5 mm / 5 min and ≤ 10 mm / 5 min should use sodium fluorescein staining method to examine the cornea and conjunctiva, positive staining (≥ 5 points) can diagnose dry eye.

Diagnosis of traditional Chinese medicine: according to the textbook ophthalmology of traditional Chinese Medicine published by Peng Qinghua in 2016 for the 13th five year plan of the national traditional Chinese medicine industry, the diagnostic basis for each syndrome type of dry eye is formulated as follows: ① deficiency of lung yin syndrome: main symptoms: dry and unsmooth eyes, unsustainable vision, white eyes as usual or slightly red pulse, black eyes may have fine spots and nebulae, which are difficult to heal repeatedly; Secondary symptoms: dry mouth, dry nose, dry throat, constipation; Tongue vein: thin moss, weak pulse. ② Liver meridian stagnation heat syndrome: main symptoms: dry eyes, burning pain, or white eyes slightly red, or black eyes star Yi, or not durable vision; Secondary symptoms: bitter mouth, dry throat, irritability, insomnia, dreaminess, dry stool or yellow urine; Tongue vein: red tongue, thin yellow or thick fur, smooth pulse string. ③ Deficiency of both qi and Yin: main symptoms: dryness and displeasure in the eyes, dryness and lack of luster in the eyes, frequent blinking of the eyes, shyness and fear of light, pale red eyes, unsustainable vision, and aggravation after long-term vision; Secondary symptoms: dry mouth, dry and cracked lips, mental fatigue. Tongue pulse: the tongue is red and the pulse is weak. ④ The main symptoms are: after suffering from wind heat or Tianxing red eye, slight photophobia and tears, a little eyesight, dryness, white eyes with a little red silk and veinlets, and slight redness in eyelids; Tongue vein: red tongue, thin yellow moss, pulse number. The above syndrome types have two or more main symptoms and secondary symptoms, and can be differentiated into different syndrome types of dry eye according to tongue and pulse.

Inclusion criteria: the patients must meet the above diagnostic criteria, and have the following conditions: ① age 18-75 years old; ② No history of drug allergy; ③ Sign informed consent.

排除标准：

①合并眼睑异常，如眼睑缺损、眼睑内外翻、眼睑闭合不全等；②先天性无泪症、先天性无泪腺、泪道阻塞和急慢性泪囊炎、眼球突出、角结膜化学伤、角膜斑翳及白斑、青光眼、增生性玻璃体视网膜病变等；③眼前段急性炎症者，如急性睑腺炎、病毒性睑皮炎、急性结膜炎、角膜炎、葡萄膜炎等；④合并有严重心血管疾病、血液病、肝肾功能不全及血糖药物控制不理想的糖尿病者，Sjgren综合征、系统性红斑狼疮者；⑤近3个月内曾接受眼部手术者；⑥近1个月内使用过影响泪液分泌的药物或者人工泪液患者；⑦妊娠或哺乳期患者。

Exclusion criteria：

① Combined with eyelid abnormalities, such as eyelid defect, entropion and ectropion, eyelid insufficiency, etc; ② Congenital absence of lacrimal gland, obstruction of lacrimal passage, acute and chronic dacryocystitis, exophthalmos, chemical injury of cornea and conjunctiva, corneal pannus and leukoplakia, glaucoma, proliferative vitreoretinopathy, etc; ③ Acute anterior segment inflammation, such as acute blepharitis, viral eyelid dermatitis, acute conjunctivitis, keratitis, uveitis, etc; ④ The patients with severe cardiovascular disease, blood disease, liver and kidney insufficiency and poor blood glucose control were diabetic, Sjgren syndrome and systemic lupus erythematosus; ⑤ Those who have undergone eye surgery in the past 3 months; ⑥ Patients who have used drugs or artificial tears that affect tear secretion in the past 1 month; ⑦ Pregnant or lactating patients.
