研究目的：

本研究比较通过膈肌超声评估拔管时间与单凭临床征象评估拔管时间的结局有无差异，意在证明通过膈肌超声评估拔管时间比单凭临床征象评估拔管时间更可靠，更有利。  

Objectives of Study：

This study compares whether there is a difference in outcome between the assessment of extubation time by diaphragmatic ultrasound and the assessment of extubation time by clinical signs alone, with the intention of demonstrating that the assessment of extubation time by diaphragmatic ultrasound is more reliable and beneficial than the assessment of extubation time by clinical signs alone.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1.2021年6月-2021年12月在昆明市儿童医院择期行四肢手术的全麻插管患；
2.ASA I-II级，年龄3~7岁。

Inclusion criteria

1. Children with general anesthesia intubation for elective extremity surgery at Kunming Children's Hospital from June 2021 to December 2021.
2. ASA I-II, aged 3-7 years.

排除标准：

1、术前患儿及家属拒绝参与及签署知情同意书者；
2、发育异常、营养不良患儿；
3、已存在膈肌损伤或合并重症肌无力、神经肌肉系统性疾病患儿；
4、苏醒后配合差，资料收集不全的患儿；
5、BMI>25kg/m^2的患儿；
6、伴有呼吸系统疾病患者。

Exclusion criteria：

1.The child and family refuse to participate and sign an informed consent form preoperative;
2.Children with developmental abnormalities and malnutrition;
3.Children with pre-existing diaphragmatic injury or combined myasthenia gravis, neuromuscular diseases;
4.Children with poor cooperation after awakening and incomplete data collection;
5.BMI>25kg/m^2;
6.Children with respiratory diseases.

依据患儿出现时间的先后顺序分别标号为1、2、3、4..…...60，采用随机数字表法随机分为A、B两组，A组为临床征象拔管组，B组为嗝肌超声拔管组。

The children were labeled 1, 2, 3, 4 ......，57,58,59，60 according to the order of presentation. All of them were randomly divided into two groups A and B using the random number table, group A being the clinical signs extubation group and group B being the diaphragmatic ultrasound extubation group.
