研究目的：

分析影响急性期颅内宽颈动脉瘤破裂出血患者并发症及预后的因素，比较支架辅助栓塞与手术夹闭治疗破裂宽颈动脉瘤的疗效及经济效益。指导急性期宽颈颅内动脉瘤手术方式的选择，降低并发症，提高治愈率，降低医疗费用。  

Objectives of Study：

Analyze the factors affecting the complications and prognosis of patients with ruptured and hemorrhage of intracranial wide-necked aneurysms in the acute stage, and compare the efficacy and economic benefits of stent-assisted embolization and surgical clipping in the treatment of ruptured wide-necked aneurysms. Guide the selection of surgical methods for wide-necked intracranial aneurysms in the acute phase, reduce complications, increase cure rates, and reduce medical costs.

药物成份或治疗方案详述：

采用前瞻性观察队列研究方法。符合纳入、排除标准的急性期破裂颅内宽颈动脉瘤患者入组行支架辅助栓塞或开颅手术夹闭治疗，并收集患者的临床特点、动脉瘤特征、并发症、住院费用等数据。分别于出院前，术后6个月进行影像学评估、mRS 评分（评价预后）。根据有无出血并发症、缺血并发症，临床预后（良好、不良）进行多因素非条件 Logistic 回归分析，探讨影响急性期宽颈动脉瘤破裂出血治疗预后的积极因素及负性因素。比较介入组、手术组治疗急性期破裂宽颈动脉瘤的效果（预后评分、并发症、住院费用）。 

Description for medicine or protocol of treatment in detail：

Adopt prospective observation cohort research method. Patients with acute ruptured intracranial wide-necked aneurysms who met the inclusion and exclusion criteria were enrolled for stent-assisted embolization or craniotomy clipping treatment, and data on the clinical characteristics, aneurysm characteristics, complications, and hospitalization expenses were collected. The imaging evaluation and mRS score (to evaluate the prognosis) were performed before discharge from the hospital and 6 months after the operation. According to the presence or absence of bleeding complications, ischemic complications, and clinical prognosis (good and bad), a multivariate unconditional logistic regression analysis was performed to explore the positive and negative factors that affect the prognosis of the treatment of ruptured wide-necked aneurysm hemorrhage in the acute phase. Compare the effects of the interventional group and the surgical group in the treatment of acute ruptured wide-necked aneurysms (prognosis score, complications, hospitalization costs).  

纳入标准：

1.头颅 CT 证实急性蛛网膜下腔出血，头颅 CT 血管成像、磁共振血管成像或脑血管造影明确诊断颅内动脉瘤；
2.采用支架辅助栓塞技术的动脉瘤瘤颈大于 4mm 或瘤体瘤颈比例小于2.0mm；
3.患者或家属同意入组的。

Inclusion criteria

1.Head CT confirmed acute subarachnoid hemorrhage, head CT angiography, magnetic resonance angiography or cerebral angiography clearly diagnosed intracranial aneurysm;
2.The aneurysm neck with stent-assisted embolization technique is larger than 4mm or the ratio of the neck of the tumor is smaller than 2.0mm;
3.Patients or their family members agree to join the group.

排除标准：

1.以往介入治疗过的动脉瘤再次破裂出血；
2.夹层动脉瘤、假性动脉瘤或外伤性动脉瘤；
3.使用特别支架技术的：双支架技术、单独支架技术、冰激凌技术等；
4.多发动脉瘤，无法判断肇事动脉瘤的；
5.临床资料不完整的患者。

Exclusion criteria：

1.The aneurysm that has undergone interventional treatment in the past ruptures and bleeds again;
2.Dissecting aneurysm, pseudoaneurysm or traumatic aneurysm;
3.Use special bracket technology: double bracket technology, single bracket technology, ice cream technology, etc.;
4.There are multiple aneurysms, and the aneurysm cannot be judged;
5.Patients with incomplete clinical data.

患者确诊宽颈颅内动脉瘤破裂出血后，由医师团队（包括神经外科血管病手术医师，及神经外科介入医师）根据患者的病情情况，以及患者和家属的意愿选择支架辅助栓塞或手术夹闭治疗。其中大致的原则是大脑中动脉瘤合并颅内血肿超过30ml 的首选手术夹闭治疗，后循环动脉瘤首选介入治疗。
