研究目的：

本研究的目的主要是根据养老机构自杀意念老年人的身心特点，构建针对性心理弹性干预方案，帮助其提高生活质量，减轻抑郁情绪，克服自杀意念，重拾生活信念。  

Objectives of Study：

The present study aims to develop resilience interventions for nursing home residents who have suicidal ideation and to test the effectiveness of resilience interventions.

药物成份或治疗方案详述：

心理弹性框架模型指出，心理弹性的发展是受到多种因素影响的，其中既包括个体内部的因素，也包括外部的因素，此外，内部因素与外部因素之间的互动也非常重要。国际心理弹性研究计划由此出发，提出了“我有”、“我是”和“我能”的操作化心理弹性训练策略，目前已成为被广泛认可和借鉴应用的心理弹性干预策略。其中“我有”指的是帮助个体发现自己拥有的有利资源与外界支持，使个体获得安全感和受保护的感觉，对映的是影响心理弹性发展的外部环境中的保护因子；“我是”指的是帮助个体了解自己的优势，发现个人的内在积极力量，树立包含自主、自尊等在内的积极感觉、态度及信念等，对映的是个体内部的心理弹性保护因子；“我能”指的是帮助个体发现和培养处理生活中可能遇到的各种问题的能力，如人际交往技巧、问题分析解决能力、情绪控制能力、压力处理能力等，对映的是当个体面临困难或问题时，个体与外部环境相作用的过程。考虑到该策略的理论基础和可操作性，我们以该策略为基础，从“我有”“我是”“我能”三个基本点出发，构建了养老机构自杀意念老年人的心理弹性干预方案。干预方案共分为八周，第一周为建立信任关系阶段，第二周和第三周为“我有”干预阶段，分为“了解我所拥有的社会支持资源”与“感恩和利用我所拥有的社会支持资源”两个部分；第四周和第五周为“我是”干预阶段，分为“发掘个人优势”和“利用个人优势”两个部分；第六周和第七周为“我能”干预阶段，分为“我能正视压力”和“我能缓解压力”两个部分；第八周为总结告别阶段。干预频率为每周干预一次，每次约90分钟。另一组为对照组，对照组的干预方案为每周一次，为期八周的一对一健康教育，八次健康教育的主题分别为：“适当运动与健康养老”“合理饮食与健康养老”“食欲不振的预防与调理”“消化不良的预防与调理”“便秘的预防与护理”“失眠的预防与调理”“跌倒的预防与处理”“皮肤瘙痒的预防与护理”。

The intervention program is divided into eight weeks, the first week is the stage of building trust relationship, the second and third weeks are the stage of "I have" intervention, which is divided into two parts: "understand the social support resources I have" and "appreciate and use the social support resources I have"; the fourth and fifth weeks are the stage of "I am" intervention, which is divided into "explore personal advantages" and "use personal advantages" The sixth and seventh weeks are the intervention stage of "I can", which is divided into two parts: "I can face the pressure" and "I can relieve the pressure"; the eighth week is the conclusion and farewell stage. The frequency of intervention was once a week, about 90 minutes each time.Another group as control group, control group intervention program for once a week, eight weeks of one-to-one health education, eight health education are: the theme of the "proper exercise pension and health" "pension reasonable diet and health" "appetite the prevention and control of" "indigestion of the prevention and control" "constipation of prevention and care" "insomnia, the prevention and control of" "fall prevention and treatment of" prevention and care skin itching. 

纳入标准：

1、年龄60周岁及以上；
2、入住养老院时间达1个月以上；
3、目前存在较为持续的自杀意念（SCL-90中条目15的“在过去一周内,我有过想结束自己的生命的想法”，回答“是”；BSI-CV第六条目，“您的这种自杀想法持续存在多长时间”，回答“较长时间”或“持续存在或几乎是持续存在的”）；
4、自愿参与研究并签署知情同意书；
5、能理解本研究和干预方案。

Inclusion criteria

1. Aged 60 years or above;
2. Stay in a nursing home for more than one month;
3. Have persistent suicidal ideation currently;
4. Participate in the study voluntarily and sign informed consent;
5. Able to understand the study and intervention program.

排除标准：

1、严重的听力损害或语言表达能力障碍影响交流者；
2、重度认知功能障碍(MMSE < 9分)；
3、终末期疾病或严重的躯体症状无法参与研究者；
4、三个月内接受过心理治疗或心理干预。

Exclusion criteria：

1. Those who have serious hearing impairment or language barrier that affects communication;
2. Severe cognitive dysfunction (MMSE < 9);
3. Patients with end-stage disease or severe somatic symptoms were unable to participate in the study;
4. Received psychological treatment or intervention within three months.

由一名不参与本研究的人员使用Research Randomizer (Version 4.0)在线随机数字生成器产生随机数字序列，指定单数为干预组，双数为对照组，并将随机数字依次放入按1~68顺序编号的密封、不透光信封中，确保隐藏分配。由一名不参与随机分组的研究人员筛选符合纳入排除标准的老人，获得知情同意后，根据入组顺序依次分配信封，根据信封内的随机数字将老人分配到干预组（n=34）和对照组（n=34）。

数据采集和管理（说明：数据采集和管理由两部分组成，一为病例记录表(Case Record Form, CRF)，二为电子采集和管理系统(Electronic Data Capture, EDC)，如ResMan即为一种基于互联网的EDC：

由经过培训的研究人员进行问卷调查，调查问卷当场回收并检查填写情况，如有漏填项，当场询问被试并进行补充，确保问卷填写的完整性；数据收集完成后，采用EpiData3.1进行双人录入数据，运用统计软件进行逻辑检测，排除由于主观原因造成的录入错误，确保资料录入的正确性。问卷和数据库由专人保管。

Data collection and Management (A standard data collection and management system include a CRF and an electronic data capture：

Trained researchers will conduct a questionnaire survey. The questionnaire will be recovered on the spot and the filling conditions will be checked. If there are missing items, the subjects will be questioned on the spot and supplemented to ensure the completeness of the questionnaire filling.After data collection, EpiData3.1 was used for double-entry data, and statistical software was used for logic detection to eliminate the entry errors caused by subjective reasons and ensure the accuracy of data entry.The questionnaire and database shall be kept by special personnel.
