研究目的：

主要研究目的：本研究通过基于网络的失眠认知行为疗法来干预抑郁障碍合并失眠障碍人群的失眠问题，以期改善青年人群的失眠症状和抑郁症状；次要研究目的：观察基于网络的失眠认知行为疗法对失眠障碍青年人群的焦虑症状、自杀观念等心理健康问题以及认知功能的改善作用，以及失眠障碍和抑郁障碍缓解率。  

Objectives of Study：

This study aims to conduct a randomized control trial to explore whether e-based CBT-I (eCBT-I) could improve depression and insomnia severity in youths with comorbid insomnia and depression and whether eCBTI could lead to more remission from insomnia disorder and major depressive disorder respectively. In addition, this study will further evaluate whether the effects of eCBT-I are generalized on other other psychopathologies such as anxiety and suicidality known to be related to insomnia, and whether eCBT-I will increase rates of remission in insomnia disorder and major depressive disorder

药物成份或治疗方案详述：

本课题为随机对照试验，将符合纳入标准的抑郁障碍合并失眠障碍的青年患者随机分为基于网络的失眠认知行为疗法组和睡眠健康教育组两组，两组受试者均进行为期6周的干预，但干预的内容不同，其中基于网络的失眠认知行为治疗组的主要内容包括认识睡眠、睡眠限制、刺激控制、认知治疗、赶走担忧和预防复发；而健康教育组的主要内容包括睡眠科普知识和健康相关知识（如睡眠卫生教育和营养学知识科普），但不包括基于网络的失眠认知行为疗法的任何有效治疗成分。且分别于基线期、治疗后以及随访1个月和随访6个月后进行评估。 

Description for medicine or protocol of treatment in detail：

This is a randomized, assessor-blind, parallel group-controlled trial conducted in Chinese youths with insomnia disorder and major depressive disorder. All eligible participants were randomly assigned to 6-week e-CBT-I or e-based health education (e-HE). The e-CBT-I intervention consists of six consecutive sessions with animated elements, including an overview of sleep, sleep restriction, stimulus control, cognitive therapy, structured worry time and relapse prevention. The e-HE is a health promotion programme with same contact hours as e-CBT-I and contains information about general sleep and health related knowledge without any active insomnia therapeutic components. Assessments were conducted at baseline, post-intervention (week 7/at the conclusion of last session), 1-month and 6-month follow-up after intervention.  

纳入标准：

（1）汉族，年龄在15-25岁的青年；
（2）符合国际疾病分类第十次修订本（International Statistical Classification of Diseases and Related Health Problems10th Revision, ICD-10）失眠障碍的诊断标准，且失眠指数量表评分（Insomnia Severity Index, ISI）10分以上；
（3）简明国际神经精神访谈（Mini-International Neuropsychiatric Interview, MINI）结果为抑郁障碍，且病人健康问卷抑郁量表（Patient Health Questionnaire 9-item, PHQ-9）评分大于等于10分；
（4）能够使用智能手机；
（5）同意参加本研究，并签署知情同意书。

Inclusion criteria

(1) Chinese Han youths aged 15-25 years old;
(2) Presence of insomnia disorder according to International Statistical Classification of Diseases and Related Health Problems10th Revision (ICD-10) diagnostic criteria and Insomnia Severity Index (ISI) > 10;
(3) Presence of current major depressive disorder as determined by the Mini-International Neuropsychiatric Interview (MINI) and Patient Health Questionnaire 9-item (PHQ-9) >=10;
(4) Accessibility to smartphones;
(5) Agree to participate in this research and sigh the consent form.

排除标准：

（1）倒班工作者；
（2）MINI访谈结果为中、高自杀风险的对象；
（3）符合精神病、精神分裂症、双相情感障碍和神经发育型障碍诊断标准；
（4）伴有湿疹、胃食管反流病等可导致睡眠障碍的躯体疾病；
（5）除失眠障碍以外的能够导致睡眠连续性中断的睡眠障碍，例如阻塞性睡眠呼吸暂停；
（6）于最近两个月更换过抗抑郁药；
（7）正在接受针对睡眠问题的心理治疗；
（8）正在接受无抽搐电休克治疗；
（9）处于怀孕和哺乳期的受试者。

Exclusion criteria：

(1) Shift workers;
(2) Presence of moderate or high risk of suicide determined by MINI;
(3) A diagnosis of psychosis, schizophrenia, bipolar disorders, or neurodevelopmental disorders;
(4) Medical conditions that interfere with sleep quality and sleep continuity, such as eczema, gastro-oesophageal and reflux disease;
(5) An additional sleep disorder (other than insomnia) that may potentially contribute to a disruption in sleep continuity and quality, such as obstructive sleep apnea;
(6) Change in antidepressant medication within the past 2 moths;
(7) Currently receiving psychological treatment for insomnia;
(8) Currently receiving electroconvulsive therapy (ECT);
(9) Women during pregnancy or lactation.
