研究目的：

评估基于快速康复外科理念的局麻下经皮肾镜取石术治疗上尿路结石的可行性、安全性和有效性及局麻对PCNL患者快速康复围术期管理的的影响。  

Objectives of Study：

To evaluate the safety and effectiveness of percutaneous nephrolithotomy under local anesthesia based on the concept of enhanced recovery after surgery, in the treatment of upper urinary calculi and the impact of local anesthesia on the perioperative management of rapid rehabilitation in PCNL patients.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1.年龄在 18～80岁间；
2.肾盏结石、单发肾盂结石 <3.5 cm；
3.L4水平以上的输尿管结石 >1.5 cm；
4.轻度或以上积水；
5.术前经医院伦理委员会批准并获得家属知情同意。

Inclusion criteria

1. Aged between 18 and 80 years;
2. Caliceal calculi and single pelvis calculi < 3.5cm;
3. Ureteral calculi above L4 level >1.5 cm;
4. Mild or greater water accumulation;
5. Preoperative approval by the ethics committee of the hospital and informed consent of the family members.

排除标准：

1.复杂结石，预估时间大于2小时；
2.合并肝肾功能不健全者、造血系统、心脑血管、肾及肝等原发性疾病，ASA评分大于3分；
3.术中无法配合体位者，包括：脊柱畸形、肺功能差等；
4.患者自我评估，疼痛耐受能力差。

Exclusion criteria：

1. Complex calculi with an estimated time of more than 2 hours;
2. Patients with combined liver and kidney dysfunction, hematopoietic system, cardiovascular and cerebrovascular diseases, kidney and liver and other primary diseases, with ASA score greater than 3 points;
3. Inoperative inability to coordinate the position, including: spinal deformity, poor lung function, etc.;
4. Patients self-assessed poor pain tolerance.
