构建基于循证的老年COPD患者的出院准备方案，进行实证应用，探索该出院准备方案的应用效果，为老年COPD患者制定全面的出院准备服务提供参考依据，推动临床护理质量持续改进。  

Objectives of Study：

To construct an evidence-based discharge preparation plan for elderly patients with COPD, conduct empirical application, and explore the application effect of the discharge preparation plan, so as to provide a reference for the development of comprehensive discharge preparation service for elderly patients with COPD, and promote the continuous improvement of clinical nursing quality.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

根据COPD全球倡议2020年最新指南诊断为COPD且因病情急性加重入院的患者；年龄≥60 周岁的蚌埠市常住居民；无语言、意识障碍，自愿参与研究并填写知情同意书。

Inclusion criteria

Patients diagnosed with COPD in accordance with the latest COPD Global Initiative 2020 guidelines and admitted to hospital with an acute exacerbation of the disease;Bengbu permanent residents aged 60 or above;No language or consciousness barrier, volunteer to participate in the study and fill in the informed consent.

排除标准：

合并心、肝、肾等功能严重障碍者；严重听视力障碍、肢体活动功能受损者；以往参加过同类型干预实验的研究者。

Exclusion criteria：

Complicated with heart, liver, kidney and other serious dysfunction;Severe hearing and visual impairment and impaired limb mobility;Who have participated in previous trials of the same type of intervention.
