研究目的：

（1）证明Liu-POEM治疗贲门失弛缓症的安全性与有效性； （2）比较Liu-POEM与传统POEM的优缺点，包括：手术时间，粘膜愈合时间，术中止血次数，术后疼痛持续时间，住院时间，并发症及复发率（6个月随访）。  

Objectives of Study：

(1) To demonstrate the safety and efficacy of Liu-POEM for the treatment of Achalasia; (2) To analyze the advantages and disadvantages of Liu-POEM and conventional POEM, including of operation time, submucosal tissuhealing time, frequency of regular hemostasis, eduration of postoperative pain, length of hospital stay, complications and recurrence in short-term follow-up (6months).

Description for medicine or protocol of treatment in detail：

 

纳入标准：

年龄为18周岁以上、80岁以下，性别不限，体重指数＜40kg/㎡；贲门失弛缓诊断标准：⑴临床表现：Eckardt评分>3(吞咽困难，反流，胸骨后疼痛，体重减轻)；⑵食管X线检查有以下表现均支持贲门失弛缓症诊断：食管扩张；食管胃结合处（EGJ）狭窄，呈“鸟嘴征”；食管蠕动消失；食管钡餐排空功能差。且无重度贲门失弛缓者（X线下食管扩张屈曲呈S形）。

Inclusion criteria

Patients with the age between 18 and 80, body mass index less than 40kg/㎡;
The diagnosis of achalasia was based on clinical symptoms(Eckardt score>3): dysphagia,regurgitation, chest pain, weight loss;
Barium esophagogram demonstrating consecutive dilatation of the esophageal body, typical 'bird's peak' appearance of the LES with decrease in emptying times;
Patients with no severe achalasia (X-ray showed expansion and s-shaped tortuosity of the lower esophagus).

排除标准：

（1）<18周岁或者>80周岁的患者； （2）不签署知情同意书； （3)不能耐受全麻； （4）妊娠期及哺乳期妇女； （5）长期使用激素患者或接受抗凝治疗存在严重出血倾向患者； （6）预期寿命<6个月，无法达到6个月随访期者（恶性肿瘤或其他严重疾病）； （7）严重肺功、肾功不全者（血浆肌酐>200 μmol/l）； （8）重度贲门失弛缓者（X线下食管扩张屈曲呈S形）； （9）肝硬化门静脉高压 ;（10）食管或近贲门处恶性肿瘤;（11）已经纳入其它研究可能对本研究有影响的患者

Exclusion criteria：

(1) Patients under the age of 18 years over 80;
(2) Patients refused to sign the informed consent;
(3) inability to tolerate general anesthesia;
(4) Pregnant women or lactating women;
(5) Long-time use of hormone therapy or anticoagulant therapy;
(6) Patients with a life expectancy less than 6 month;
(7) severe pulmonary disease and renal insufficiency (Blood creatinine >200 umol/L);
(8）severe achalasia (X-ray showed expansion and s-shaped tortuosity of the lower esophagus);
(9) cirrhosis with portal hypertension;
(10) patients have concomitant esophageal or upper gastric tumors;
(11) Patient who has took part in other studies that can affect results of the present study.
