Objectives of Study：

The effect of alkalized urine on kidney stones in patients with gout

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

（1）年龄＞18岁，＜80岁；
（2）符合痛风诊断（2015年ACR/EULAR痛风分类标准）；
（3）B超提示肾结石；
（4）男性近6个月无生育要求；
（5）绝经后女性(停经2年)，或手术节育；
（6）同意参加本研究,并签署知情同意书。

Inclusion criteria

(1) Age> 18 years old, <80 years old;
(2) Meet the diagnosis of gout (2015 ACR/EULAR gout classification criteria);
(3) B-ultrasound suggests kidney stones;
(4) Males have no childbearing requirements in the past 6 months;
(5) Postmenopausal women (menopause for 2 years), or surgical birth control;
(6) Agree to participate in this study and sign an informed consent form.

排除标准：

（1） 入组前4周内使用过别嘌呤醇、丙磺舒、苯溴马隆、非布司他等降尿酸；
（2） 肾结石大于15mm、梗阻（肾盂积水）、疼痛、泌尿系感染需要排石处理[10]；
（3） 由肾脏病、血液病，或服用某些药物、肿瘤放化疗、器官移植等原因引起的继发性高尿酸血症者；
（4） 曾有中风、TIA、MI、HF（NYHA分级II-IV）、冠状动脉手术（如血管成形术、支架植入术、旁路移植术等）等心脑血管病史者；
（5） 曾有消化道溃疡、消化道出血病史者；
（6） 肝病活动期或肝功能异常，AST、ALT或GGT高于正常值上限1.2倍者；
（7） 肾功能异常，简化肾小球滤过率(eGFR)＜30ml/min.1.73m2者；
（8） 合并恶性肿瘤或精神病者；
（9） 对非布司他过敏或不耐受者；
（10） 其他（非痛风）慢性关节炎、急性炎症性关节炎、伴有关节炎的自身免疫性疾病；
（11） 正在或需要服用硫唑嘌呤、巯嘌呤、茶碱和细胞毒类化疗药物者；
（12） 有酒精或药物依赖史，或任何原因需要长期每日使用止痛药者；
（13） 入组前3个月内参加其他临床研究者；
（14） 具有其他各种研究者认为不宜参加该项临床研究的情况。

Exclusion criteria：

(1) Allopurinol, probenecid, benzbromarone, febuxostat, etc. have been used to lower uric acid within 4 weeks before enrollment;
(2) Kidney stones larger than 15mm, obstruction (hydronephrosis), pain, and urinary tract infections need to be treated with stones[10];
(3) Patients with secondary hyperuricemia caused by kidney disease, blood disease, or taking certain drugs, tumor radiotherapy and chemotherapy, organ transplantation, etc.;
(4) People with a history of cardiovascular and cerebrovascular diseases such as stroke, TIA, MI, HF (NYHA class II-IV), coronary artery surgery (such as angioplasty, stent implantation, bypass grafting, etc.);
(5) Those who have had a history of digestive tract ulcers and gastrointestinal bleeding;
(6) Patients with active liver disease or abnormal liver function, with AST, ALT or GGT higher than 1.2 times the upper limit of normal;
(7) Abnormal renal function, simplified glomerular filtration rate (eGFR) <30ml/min.1.73m2;
(8) Patients with malignant tumors or mental illness;
(9) Those who are allergic to or intolerant to febuxostat;
(10) Other (non-gout) chronic arthritis, acute inflammatory arthritis, autoimmune diseases accompanied by arthritis;
(11) Those who are or need to take azathioprine, mercaptopurine, theophylline and cytotoxic chemotherapy drugs;
(12) People who have a history of alcohol or drug dependence, or need long-term daily use of analgesics for any reason;
(13) Participate in other clinical research within 3 months before enrollment;
(14) There are situations in which various other researchers think it is inappropriate to participate in the clinical research.
