研究目的：

利用新研发的手术导航系统，进行腰椎间盘突出症及椎间盘源性腰痛的精准靶向射频消融治疗，并验证该系统的安全性和有效性。  

Objectives of Study：

The newly developed surgical navigation system was used for precise targeted radiofrequency ablation for lumbar disc herniation and discogenic low back pain, and the safety and effectiveness of the system were verified.

药物成份或治疗方案详述：

射频消融术作为一种新的微创介入治疗方式,其具有创伤小、出血少、手术时间短及术后恢复快等特点，该技术原理是利用射频电场于刀头周围产生等离子薄层，使离子获得足够动能，打断分子键，使生物大分子分解成小分子或低分子气体，再调节刀头温度，对离子化的髓核组织进行热凝，起到紧缩、止血作用。气化髓核组织后，椎间盘内压力降低，灭活椎间盘内致痛神经，减轻炎症反应，使突出椎间盘自主回缩，从而治疗腰椎间盘突出症及椎间盘源性腰痛。 

Description for medicine or protocol of treatment in detail：

Radiofrequency ablation as a new minimally invasive interventional treatment, it has small trauma, less bleeding, shorter operation time and postoperative recovery fast and other characteristics, the technical principle is the use of radio frequency electric field in a plasma around a bit thin layer, and the ions get enough kinetic energy, interrupt molecular bonds, making biological macromolecules into smaller molecules or low molecular gases, then adjust the temperature of the cutting tools,Heat coagulation to ionized nucleus pulposus tissue, contraction, hemostatic effect.After vaporization of the nucleus pulposus tissue, the pressure in the intervertebral disc is reduced, the painful nerve in the intervertebral disc is inactivated, the inflammatory reaction is reduced, and the herniated intervertebral disc spontaneously retreats, thus treating the lumbar disc herniation and the discogenic low back pain. 

纳入标准：

（1）年龄均在18-65周岁之间。
（2）确诊为腰椎间盘突出症或椎间盘源性腰痛。
（3）腰椎CT或MRI检查发现明确的腰椎间盘退变或突出，并且病变节段同患者临床症状相符合。
（4）临床症状持续时间超过3个月，或虽初次发作但疼痛剧烈，严重影响生活质量。
（5）保守治疗效果不佳，或临床症状反复发作。

Inclusion criteria

（1）age between 18 years to 65 years.
(2) Diagnosed as lumbar disc herniation or discoenic low back pain.
(3) Lumbar CT or MRI examination found clear lumbar disc degeneration or herniation, and the lesion segment was consistent with the patient's clinical symptoms.
(4) The duration of clinical symptoms is more than 3 months, or although the first attack but severe pain, seriously affecting the quality of life.
(5) Poor conservative treatment effect, or recurrent clinical symptoms.

排除标准：

1） 合并可能会导致术中出血量增多、手术时间增加的血液系统疾病患者或者有近期服用抗凝药物者。
（2） 既往有腰椎手术史的患者。
（3）合并有腰椎管狭窄、腰椎滑脱、腰椎肿瘤病变的患者。
（4）有马尾神经综合征相关症状的患者。
（5）手术部位皮肤感染
（6）认知功能受限或精神疾患无法配合治疗

Exclusion criteria：

1) Patients with blood system diseases that may lead to increased intraoperative blood loss and longer operation time or those who have taken anticoagulant drugs recently.
(2) Patients with a history of lumbar spine surgery.
(3) Patients with lumbar spinal stenosis, lumbar spondylolisthesis and lumbar tumor lesions.
(4) Patients with symptoms related to cauda equina syndrome.
(5) Skin infection at the surgical site

盲法：

双盲，不对患者讲解采用导航引导穿刺射频治疗或是X光机引导穿刺射频治疗，进行随访数据采集的医生不参与分组安排，不进入手术室参与手术，不了解治疗分组情况。

Blinding：

Double-blind, patients were not explained to use navigation-guided radiofrequency puncture therapy or X-ray machine-guided radiofrequency puncture therapy. Doctors who performed follow-up data collection did not participate in grouping arrangement, did not enter the operating room to participate in surgery, and did not understand the treatment grouping.

数据采集和管理（说明：数据采集和管理由两部分组成，一为病例记录表(Case Record Form, CRF)，二为电子采集和管理系统(Electronic Data Capture, EDC)，如ResMan即为一种基于互联网的EDC：

研究者在诊治受试者后及时填写CRF，保证病例报告表上的内容与门诊或住院病历上的内容一致。 病例报告表做任何更正时只能划线，旁注改后的数据，由研究者签名并注明日期，不得擦涂、覆盖原始记录。 门诊及住院受试者的原始化验单均可在医院的HIS系统上溯源，其化验结果均需填写至病例报告表的相应的表格里。 使用ResMan作为本研究的EDC

Data collection and Management (A standard data collection and management system include a CRF and an electronic data capture：

The investigator filled in the CRF in time after diagnosis and treatment to ensure that the content on the case report form was consistent with the content on the outpatient or inpatient medical records. Any corrections to the case report form should only be underlined, and the revised data should be sidelined, signed and dated by the investigator, and the original records should not be erased or overwritten. The original test sheets of both outpatient and inpatient subjects can be traced to the HOSPITAL'S HIS system, and the test results should be filled in the corresponding form in the case report form. ResMan was used as the EDC of this study.
