研发自身重力牵引装置,将水平机械加载的预负荷完全转化为自身重力控制和量化的牵引力,完成对牵引力的精确控制和量化,在此基础上采用称重传感器技术及红外线测量等方法进行在体力学测量,获得持续自重牵引过程中体外力学变化情况,通过计算机同步记录;以此为边界条件,利用三维有限元模型分析持续不同量自重牵引力加载情况下模型内部应力及位移变化情况;通过在体力学测量和有限元分析实验数据对比和相互验证,揭示持续自重牵引治疗腰痛的生物力学机制,明确最佳牵引剂量和牵引参数,以证实腰椎牵引治疗腰痛的正向作用。 |

Objectives of Study: |
The research team has developed a self-weight traction device that fully converts the preload of horizontal mechanical loading into self-weight control and quantified traction force and completes the precise control and quantification of traction force. On the foundation of the device, the load cell technology and infrared measurement, and other methods are used to perform in-body mechanics measurements to obtain the external mechanical changes during continuous self-weight traction. Data are recorded synchronously by the computer. As a boundary condition, the three-dimensional finite element model is used to analyze the internal stress and displacement changes of the model under continuous loading with different amounts of self-weight traction. By comparing and mutually verifying experimental data through in-mechanical measurement and finite element analysis, the biomechanical mechanism of using continuous self-weight traction to treat low back pain is revealed. And, the optimal traction dose and traction parameters are pointed out. Besides, the positive effects of lumbar traction on LBP (low back pain) are verified. |

①年龄在 20-50 岁之间;②症状均为超过 3 个月的慢性 LBP 伴坐骨神经痛;③自愿并签署知情同意书。 |

Inclusion criteria |
①Age between 20-50 years old; ②Symptoms are chronic LBP with sciatica over 3 months; ③Voluntary and signed informed consent. |

排除标准: |
①由肿瘤、感染或骨折引起的 LBP;②已行腰部手术治疗; ③有神经系统缺陷性疾病;④重度骨质疏松症及精神疾病患者,如抑郁症、焦虑 症等;有肝肾功能不全者;存在严重心肺功能不全疾病的患者;已怀孕患者;同 意参加试验之前 6 个月内有脑血管意外和/或心肌梗塞病史的患者;⑤腰椎 MRI/CT 检查指示腰椎间盘脱出(脱垂或上翘)或钙化的患者。 |

Exclusion criteria: |
① LBP caused by tumor, infection or fracture; ② Has been treated with lumbar surgery; ③ Has neurological defect disease; ④ Patients with severe osteoporosis and mental illness, such as depression, anxiety, etc.; have liver and kidney dysfunction Patients; patients with severe cardiopulmonary insufficiency; patients who are pregnant; patients with a history of cerebrovascular accident and/or myocardial infarction within 6 months before agreeing to participate in the trial; ⑤Lumbar MRI/CT examination indicates lumbar intervertebral disc prolapse (prolapse or Upturned) or calcified patients. |

完善患者选择标准,选取单阶段单侧 LDH 患者 120 例, 确定基线变量一致,随机分配到假牵引组、牵引组 1 及牵引组 2,均使用自身重 力牵引装置,将水平机械加载的预负荷等量转化为自重牵引力,分别采用 20%体 重、40%体重、60%体重的自重牵引力,设定持续牵引时间为最初 3 周 10min,后 3 周 20min,3 次/周,共 6 周。所有均有指定医生完成。 |
