研究目的：

比较传统有创机械通气的撤机判断方法及膈肌收缩压力指数判断撤机方法在临床应用价值。  

Objectives of Study：

The clinical application value of traditional invasive mechanical ventilation disengagement method and diaphragm systolic pressure index disengagement method were compared.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1. 2018年03月01日-2020年03月入住皖南医学院弋矶山医院重症医学科经口气管插管行有创机械通气患者；
2. 年龄≥18岁；
3. 气管插管和机械通气治疗超过24h以上，且成功通过SBT试验的患者。

Inclusion criteria

1. Patients admitted to the Icu of Yijishan Hospital, Wannan Medical College from March 1, 2018 to March 2020 who underwent invasive mechanical ventilation through airway intubation;
2. Aged >= 18 years;
3. Patients who were treated with endotracheal intubation and mechanical ventilation for more than 24 hours and successfully passed the SBT test.

排除标准：

1. 妊娠；
2. 气管切开；
3. 膈肌麻痹患者。

Exclusion criteria：

1. Pregnancy;
2. Tracheotomy;
3. Patients with diaphragmatic paralysis.

金标准或参考标准（即可准确诊断某疾病的单项方法或多项联合方法，在本研究中用于诊断是否有该病的临床参考标准）：

气管插管患者拔管后48h内无呼吸机辅助通气（包括无创呼吸机）。

Gold Standard or Reference Standard (The clinical reference standards required to establish the presence or absence of the target condition in the tested population in present study):

Non-ventilator assisted ventilation (including non-invasive ventilator) within 48 hours after extubation for patients with endotracheal intubation.

指标试验（即本研究的待评估诊断试验，无论为方法、生物标志物或设备，均请列出名称）：

膈肌收缩压力指数；膈肌厚度变异度；膈肌位移；最大吸气负压。

Index test:

Diaphragm contraction pressure index (DCPI)；diaphragm thickening fraction (DTF)；diaphragmatic inspiratory excursion (DE) ；maximum inspiratory pressure (MIP).

目标人群（可以是某种疾病患者或正常人群，详细描述其疾病特征，注意应纳入符合分布特点的全序列病例，具有良好的代表性）

气管插管≥24h且成功拔管的患者。

拔管前按随机数字表法，依据本研究的纳入标准与排除标准，将符合标准的240名研究对象按照纳入研究的时间进行编号，采用随机数字表法随机分组，研究对象入组后由专人打开对应编号的信封，跟患者拔管结果完成分组工作。

Before extubation, according to the inclusion and exclusion criteria of this study, 240 subjects meeting the criteria were numbered according to the time of inclusion in the study and randomly grouped by the random number table method.
