研究目的：

通过多中心随机对照观察300例慢性尿路感染患者，在慢性尿路感染稳定期或在急性发作期采用敏感抗菌治疗2周后，分为两组,西药组予以长程抑菌疗法，中药组辨证治疗(按偏肾阳虚、偏肾阴虚两种证型分别采用二丁二仙汤、滋肾清利方治疗，并随症加味)，评估中医药辨证治疗中老年女性慢性尿路感染（再发性泌尿道感染）的临床疗效，重在观察对中老年女性慢性泌尿道感染的复发率及对小管间质损伤的作用，以及从心理、社会等功能状态观察中医药改善生命质量的作用,建立慢性泌尿道感染的中西医结合治疗方案——在急性期敏感抗菌素治疗结合缓解期中药辨证治疗方案。  

Objectives of Study：

300 patients with chronic urinary tract infection at the stable stage or after acute stage during which, two-week sensitive antibiotics treatment have already been used, enrolled from multi-centers, are randomly assigned into two groups. Western medicine group: the patients are treated with long-term antibiotic therapy; T.C.M. group based on syndrome differentiation: the patients differentiated as kidney Yang deficiency syndrome, are treated with Er Ding Er Xian decoction. At the same time, the patients differentiated as Kidney Yin deficiency syndrome, are treated with nourishing kidney and eliminating dampness decoction. Then, to evaluate clinical effect of T,C.M, treatment on chronic urinary tract infection female patients at middle and eld age, the effect on the relapse rate of UTI and renal tubular injury are assessed. In addition, the effect on improving quality of life, including mental and social statusare also measured. Taken together, we try to establish an integrative therapy based on TCM and Western medicine for treating chronic urinary tract infection. That is to say, the treatment is antibiotics therapy at acute stage combined with TCM treatment based on syndrome differentiation during remittence period.

药物成份或治疗方案详述：

偏肾阳虚者，应用二丁二仙汤：由仙茅15克、淫羊藿15克、当归15克、黄柏15克、知母15克、蒲公英30克、地丁草30克、红藤30克组成； 偏肾阴虚者，应用滋肾清利方：太子参15克、生地15克、牛膝15克、米仁15克、知母15克、蒲公英30克、地丁草30克、红藤30克组成； 在上述辨证选方基础上，再进行随症加味： 尿频、尿急症状明显，加穿心莲15克、车前子15克； 乏力、神疲明显，加山药15克、黄芪15克； 小腹胀痛明显，加乌药10克、香附10克； 腰部刺痛或舌暗有瘀斑，加桃仁10克、虎杖15克。 水煎剂，每日1剂，分2次口服。 

Description for medicine or protocol of treatment in detail：

Patients differentiated as "Kidney Yang deficiency syndrome" are treated with Er Ding Er Xian decoction, which includes, curculiginis 15g, herba erimedii 15g, angelica 15g, phellodendri cortex 15g, anemarrhenae 15g, dandelion 30g, viola herba 30g, sargentgloryvine stem 30g. Patients differentiated as "Kidney Yin deficiency syndrome" are treated with nourishing kidney and eliminating dampness decoction, which includes, pseodostellariae 15g, rehmannia dried rhizome 15g, achyranthes 15g, semen coicis 15g，anemarrhenae 15g, dandelion 30g, viola herba 30g, sargentgloryvine stem 30g. In addition, other herbs are added into these decoctions based on different symptoms: For symptoms like urinary frequency and urgency, the decoction are added with andrographitis 15g, plantains semen 15g. For symptoms like hypodynamia, the decoction are added with batatatis 15g, astragalus 15g. For symptoms like lower abdominal distention, the decoction are added with radices linderae 10g, rhizoma cyperi 15g. For symptoms like loin stitch or ecchymosis in the tongne, the decoction are added with percicae semen 10g, giant knotweed rhizome 15g. The decoction is decocted in water, one a day, for oral administration twice a day. 

纳入标准：

①符合西医慢性尿路感染(再发性泌尿道感染）诊断标准；
②符合中医劳淋诊断标准；
③年龄在18－75岁之间；
④签署知情同意书。

Inclusion criteria

1. Comply with the western medical diagnostic criteria of chronic urinary tract infection (recurrent urinary tract infection);
2. Comply with the TCM diagnostic criteria of urinary tract infection;
3. Aged 18-75 years;
4. Sign the informed consent.

排除标准：

①急性肾盂肾炎或初次发作的急性尿路感染者；
②影像学检查发现尿路畸形、结石、积水等病变；
③尿道综合征；
④血肌酐≥442μmol/L的慢性肾衰竭；
⑤妊娠或哺乳妇女（育龄期女性需做尿HCG检查以确定有无妊娠）；
⑥对本试验两种对照药物有过敏史或过敏体质者；
⑦心、肝、肾功能严重损害或患糖尿病等其他严重基础疾病者；
⑧严重中枢神经系统疾病者。

Exclusion criteria：

1. acute pyelonephritis or acute urinary tract infection occurred for the first time;
2. Deformity, stone or hydrops of urethra, detected by radiological examinations;
3. Urethral syndrome;
4. Chronic renal failure, serum creatinine≥442 umol/L;
5. Pregnant or lactating (women at child-bearing period are requested to have urinary HCG test to detect pregnancy);
6. Allergic to the test drugs or ingredients of the TCM granules;
7. Severe damage of heart, liver or kidney, and other severe diseases including diabetes;
8. Severe central nervous system diseases.
