研究目的：

评估保肾方治疗2型糖尿病肾病（diabetic nephropathy，DN）IV期，属气阴两虚，瘀血阻络证患者的临床疗效和安全性。 2.2 通过观察高糖记忆标志物、足细胞调控因子、miR-21以及足细胞上皮-间充质转分化（epithelial-mesenchymal transition，EMT）相关蛋白等指标，阐明其作用机制。  

Objectives of Study：

Objective and evaluate the clinical efficacy and safety of Baoshen recipe in the treatment of type 2 diabetic nephropathy stage IV, which belongs to Qi Yin deficiency and blood stasis blocking collaterals syndrome. The mechanism of high glucose memory markers, podocyte regulatory factors, miR-21 and podocyte epithelial mesenchymal transition related proteins were observed.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1.符合T2DM诊断的患者；
2.符合西医糖尿病肾病Ⅳ期诊断标准并且合并糖尿病视网膜病变患者，而且尿蛋白≤3.5g/24h，血浆白蛋白≥30g/L，糖化血红蛋白＜8%，血肌酐＜3mg/dl；
3.符合气阴两虚，瘀血阻络证的患者；
4.年龄在18至80岁之间。

Inclusion criteria

1. Patients who meet the diagnosis of type 2 diabetes mellitus;
2. Patients with diabetic retinopathy who meet the diagnostic criteria of stage IV diabetic nephropathy in western medicine, and have urinary protein ≤ 3.5g/24h, plasma albumin ≥ 30g / L, glycosylated hemoglobin < 8%, serum creatinine < 3mg / dl;
3. Patients with Qi Yin deficiency and blood stasis obstructing collaterals syndrome;
4. The age is between 18 and 80 years old.

排除标准：

1.妊娠或准备妊娠及哺乳期妇女；
2.合并心、脑、肺、肝和造血系统等严重原发性疾病，过敏体质者；
3.无法合作者，如精神病患者；
4.存在活动性感染灶：如肺结核等；
5.各种肿瘤继发肾脏病患者及结核病等消耗性疾病患者；
6.正在参加其它药物临床试验者或12周内参加过其它临床试验者；
7.12周内用过糖皮质激素、免疫抑制剂、雷公藤制剂。

Exclusion criteria：

1. Pregnant women or women preparing for pregnancy and lactation;
2. Patients with severe primary diseases such as heart, brain, lung, liver and hematopoietic system and allergic constitution;
3. Those who can't cooperate, such as mental patients;
4. There are active infection foci, such as pulmonary tuberculosis;
5. Patients with kidney disease secondary to various tumors and patients with consumptive diseases such as tuberculosis;
6. Those who are participating in other drug clinical trials or have participated in other clinical trials within 12 weeks;
7. Used glucocorticoid, immunosuppressant and Tripterygium wilfordii preparation within 12 weeks.

随机化方案借助SAS9.4统计分析软件产生2型糖尿病肾病（Ⅳ期）受试者所接受（试验组和对照组）的随机安排，即列出流水号001~094所对应的治疗分配。

The randomized scheme was used to generate the randomized arrangement (trial group and control group) of subjects with type 2 diabetic nephropathy (stage IV) with the help of sas9.4 statistical analysis software, that is, the treatment allocation corresponding to the serial number 001 ~ 094 was listed.
