研究目的：

研究胰管支架置入对急性胆源性胰腺炎的治疗效果，并通过代谢组学分析患者ERCP联合胰管支架置入治疗前后的代谢物构成特征谱，初步探讨代谢物与治疗之间的关系。  

Objectives of Study：

To study the effect of pancreatic stenting on the treatment of acute gallstone pancreatitis, and analyze the metabolite composition profile of patients before and after the treatment of ERCP combined with pancreatic duct stenting through metabonomics, and to preliminarily explore the relationship between metabolites and treatment .

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1.年龄18～70岁；
2.发病时间≤72h；
3.APACHE II评分≥8分或CRP≥150mg/L；
4.符合急性胆源性胰腺炎诊断标准；
5.签署知情同意书。

Inclusion criteria

1. Age from 18 to 70 years old;
2. Onset time ≤72h;
3. APACHE II score ≥ 8 points or CRP ≥ 150 mg/L;
4. Meet the diagnostic criteria for acute biliary pancreatitis;
5. Sign the informed consent form.

排除标准：

1.合并急性胆管炎或胆道梗阻的患者；
2.既往接受胰腺手术的患者；
3.孕妇或哺乳期妇女；
4.慢性胰腺炎急性发展期；
5.合并严重心、肺、肝、肾、精神疾病影响急性胰腺炎治疗效果或无法配合完成操作者（心功能＞III级，急性心梗，慢性肾病肌酐清除率＜40ml/min，需要长期氧疗的慢阻肺患者，慢性肝病肝功能Child-Pugh C级，免疫疾病或免疫抑制状态，濒死患者）；
6.各种原因不能耐受胰管支架置入患者（如造影剂过敏、重度胃底食管静脉曲张；上消化道梗阻、溃疡、出血等；凝血功能障碍或长期口服抗凝药物；病人或家属拒绝等）；
7.合并胆胰系统恶性肿瘤影响治疗效果者；
8.合并其他疾病需同期手术治疗者。

Exclusion criteria：

1. Patients with acute cholangitis or biliary obstruction;
2. Patients who have undergone pancreatic surgery in the past;
3. Pregnant or lactating women;
4. Acute development stage of chronic pancreatitis;
5. Patients with severe heart, lung, liver, kidney, or mental illness that affect the treatment effect of acute pancreatitis or cannot cooperate with the operator (heart function> grade III, acute myocardial infarction, chronic kidney disease creatinine clearance rate <40ml/min, long-term oxygen is required Patients with chronic obstructive pulmonary disease, Child-Pugh Class C liver function of chronic liver disease, immune disease or immunosuppression status, dying patients);
6. Patients who cannot tolerate pancreatic duct stent implantation for various reasons (such as contrast agent allergy, severe gastric esophageal varices; upper gastrointestinal obstruction, ulcers, bleeding, etc.; coagulation dysfunction or long-term oral anticoagulation drugs; patients or family members Refusal, etc.);
7. Patients with malignant tumors of the biliary and pancreatic system affecting the treatment effect;
8. Patients with other diseases that require simultaneous surgical treatment.
