Objectives of Study：

To assess the effects of early retroperitoneal drainage percutaneously in severe acute pancreatitis

药物成份或治疗方案详述：

纳入标准：

1. 18≤年龄≤75
2. 肠功能评分>9
3. Balthazar CT 评分达到D级或E级
4. 存在腹膜后穿刺入路
5. 签署知情同意

Inclusion criteria

1. age equal to or above 18 years, equal or less than 75;
2. Gut Dysfunction score>9;
3. Balthazar CT score=Grade D or Grade E;
4. possibility of placing a catheter into the retroperitoneal space percutaneously;
5. written informed consent.

排除标准：

1. 已经引流或行手术清创。ERCP导致的重症急性胰腺炎不排除
2. 因急腹症或为明确诊断，已行剖腹探查术
3. 急性或慢性胰腺炎病史
4. 存在恶性疾病
5. 腹腔内脏穿孔、大出血并且需要手术干预
6. 女性患者处于妊娠期
7. 精神病
8. 确诊重症急性胰腺炎距发病超过7天
9. 血细菌培养阳性

Exclusion criteria：

1. previous drainage or surgical necrosectomy for (suspected);
infected pancreatic necrosis, including procedures performed in referring hospitals. ERCP with or without papillotomy is allowed;
2. previous exploratory laparotomy for acute abdomen and diagnosis of pancreatitis during laparotomy;
3. history of acute or chronic pancreatitis;
4. bleeding, or perforation of a visceral organ as indication for intervention;
5. malignancy;
6. pregnancy;
7. mental illness;
8. identification of patients > 7days after onset of symptoms;
9 bacteremia.

随机分组后24小时内行B超引导下腹膜后积液穿刺引流。引流管直径不低于12F。穿刺路径首选经左侧腹膜后穿刺，若这里不能穿刺，则选择经腹膜穿刺，右侧腹膜后路径也可以考虑。当引流管放置第五天时，无论引流液量有多少，一律拔出引流管，以减少感染发生率。

干预措施代码：

Intervention：

A percutaneous drain (at least 12 French) is placed in the (peri-)pancreatic collection with the assistance of ultrasound, within 24 hours after randomization. The preferred route is through the left retroperitoneum. If this is not possible a transperitoneal route is chosen. A right retroperitoneal route is allowed when it can be safely applied. Catheters are removed 5 days later regardless of volume of drained collection to avoid Secondary infection.
