研究目的：

评估在腹部大手术患者中输注碳酸氢钠林格注射液与乳酸钠林格注射液术后急性肾损伤的影响  

Objectives of Study：

To evaluate the effect of Sodium Bicarbonate Ringers Injection and Sodium Lactated Ringers Injection on acute kidney injury in patients undergoing major abdominal surgery

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1) ASAⅠ-Ⅲ级；
2) 患者年龄20-70岁、签署知情同意书，愿意参加该研究患者；
3) BMI≤30kg/m2；
4) 首次接受择期腹部大手术（卵巢癌根治术、宫颈癌根治术、胃癌根治术、结直肠癌根治术、膀胱肿瘤根治术及前列腺肿瘤根治术）；
5) NYHA心功能分级Ⅰ-Ⅲ级；
6) 术前肝肾功能正常；
7) 术前凝血功能正常；
8) 血红蛋白>70g/L；
9) 心电图检测窦性心律且无其他恶性心律失常；

Inclusion criteria

1) ASA grade I to III;
2) patients aged between 20 and 70 years, signed informed consent form, and are willing to participate in the study;
3) BMI <=30kg/m2;
4) elective major abdominal surgery for the first time (radical resection of ovarian cancer, radical resection of cervical cancer, radical resection of gastric cancer, radical resection of colorectal cancer, radical resection of bladder tumor and radical resection of prostate tumor);
5) NYHA cardiac function grade I - III;
6) the function of liver and kidney is normal before operation;
7) the blood coagulation function was normal before operation;
8) hemoglobin > 70g/L;
9) sinus rhythm was detected by electrocardiogram and there were no other malignant arrhythmias.

排除标准：

1) 急诊手术；
2) 肝肾手术；
3) 合并慢性呼吸系统疾病且FEV1/FVC<70%；
4) 手术时间<2 h的手术；
5) 合并精神心理疾病或认知功能障碍患者;
6) 未经控制的高血压且血压高于180/100mmHg.
7) 甲状腺功能减退患者；
8) 妊娠或哺乳期患者。

Exclusion criteria：

1) Emergency surgery.
2) liver and kidney surgery.
3) complicated with chronic respiratory diseases and FEV1/FVC < 70%.
4) the operation time is less than 2 hours.
5) patients with mental illness or cognitive impairment.
6) uncontrolled hypertension with blood pressure higher than 180 × 100mmHg.
7) patients with hypothyroidism.
8) pregnant or lactating patients.
