研究目的：

本研究拟在湖南省基层医院建立糖尿病医药联合门诊，比较医药联合门诊与传统疾病管理模式（医生单独管理）下糖尿病管理质量的差异，进一步明确医药联合门诊是否可以提高基层医院糖尿病患者管理水平，从而更好地实现分级诊疗制度。  

Objectives of Study：

This study aimed to establish physician-pharmacist collaborative diabetes clinics in primary hospitals in Hunan Province and to compare the differences between the collaborative diabetes clinics and usal care clinics in diabetes management, and aimed to evaluate the effects of physician-pharmacist collaborative diabetes clinics in primary hospitals for the hierarchical medical system.

纳入标准：

Inclusion criteria

排除标准：

患者只要符合以下任何一条，即排除于本研究。
① 妊娠；
② 终末期肾功能衰竭（血液透析或肾小球滤过率＜10ml／min）；
③ 痴呆或精神状态不稳定；
④ 接受癌症治疗；
⑤ 有急性或慢性感染，可能影响第一个月血糖控制；
⑥ 有充血性心力衰竭，需要药物治疗；
⑦ 既往有急性胰腺炎或慢性胰腺炎史。

Exclusion criteria：

1. Pregnancy;
2. End-stage renal failure (hemodialysis or glomerular filtration rate<10mL/min);
3. Dementia or unstable psychiatric conditions;
4. Treatment for cancer;
5. Have acute or chronic infection that will affect glucose control for 1st month assessment;
6. Have congestive heart failure which requires medicine treatment;
7. Have a history of acute or chronic pancreatitis.

将全省88个县市级医院根据人口和经济水平分成低、中、高3个级别，同一别医院各随机选取2家医院，利用随机数字表将其随机分为实验组和对照组。

Randomization Procedure (please state who generates the random number sequence and by what method)：

All 88 counties in Hunan province were divided into three groups based on their population and per-capita income levels, namely high, medium and low-income counties. Two counties in each subgroup will be selected and randomly assigned to either the intervention group or the control group using Random Number.
