研究目的：

验证应用fMRI技术以及基于fMRI图像的影像组学方法定量评价局部晚期HNSCC患者放化疗疗效的可行性，探索早期预测放化疗疗效的最佳影像学指标，为局部晚期HNSCC患者个体化治疗、放化疗方案优化提供临床决策参考。

Objectives of Study：

To validate the feasibility of quantitative evaluation of efficacy of chemo-radiotherapy in locally advanced HNSCC patients by combination of fMRI techniques such as DWI, IVIM-DWI, DKI and DCE-MRI, and radiomics based on fMRI image features, and to explore the best imaging indicators for early prediction, so as to provide clinical reference for individualized treatment and optimization of chemoradiotherapy regimen for locally advanced HNSCC patients.

纳入标准：

Inclusion criteria

1. In patients with HNSCC, the primary sites included pharynx (including nasopharynx, oropharynx, hypopharynx), larynx, nasal cavity and paranasal sinuses;
2. Patients with definite pathological diagnosis and clinical staging of locally advanced tumors (according to AJCC 8th Edition head and neck tumor staging criteria: T1-4 N2-3 M0 but not T1-2 N1 M0, T3-4 n0-1 M0);
3. For subjects with KPS score >= 70, ECoG score: 0-1;
4. Subjects aged 18-70 years old;
5. Subjects who can tolerate plain and enhanced MRI examination;
6. Imaging examination (plain MRI + enhanced MRI); The range of primary lesion invasion and lymph node metastasis were confirmed by color Doppler ultrasound (CDFI);
7. Subjects with normal liver and kidney function, without severe hematologic toxicity and severe medical complications, were allowed to participate in induction chemotherapy, concurrent chemotherapy, consolidation chemotherapy, targeted therapy and other treatment methods;
8. Subjects without surgical intervention;
9. Volunteers who signed informed consent.

排除标准：

1.年龄大于70岁；
2.功能状态评分（KPS）评分＜70分，体能状态评分（ECOG）＞1分；
3.因肝肾功能不良、严重血液系统毒性及内科合并症无法行诱导化疗、同步化疗等治疗方式；
4.既往头颈部放疗史；
5.口腔有义齿、假体等金属植入物；
6.幽闭恐惧症。

Exclusion criteria：

1. Patients over 70 years old;
2. For patients whose KPS score was less than 70, ECoG score was more than 1;
3. Patients with liver and kidney dysfunction, severe hematologic toxicity and medical complications can not be treated with induction chemotherapy or synchronous chemotherapy;
4. Patients with previous head and neck radiotherapy history;
5. Patients with denture, prosthesis and other metal implants;
6. Claustrophobic patients.
